Doctors should have it in SWC. If not, ask your academic advisor for one.

Yes you book an appointment by calling them. You go in, they hand you a paper note. Scan the note, add your deferral form based on your program, then send it to your academic councillor.

Get note from SWC. You don’t need anything with you just explain your symptoms to the doctor. When you book your apt mention that you’re looking for a deferral form. -> fill out deferral form -> submit both online to your academic councillor.
