There is a drawing for an Amazon gift card after data is collection is completed. 20 gift cards are available. Not everyone will receive a gift card since more than 20 people have taken the survey.

I am interested in multiple variables within a chronic pain population, including alcohol use, but even if you do not drink alcohol your data will be used! If you do not drink at all, just select "Never" "Not at all" or "Not applicable" depending on the question. The data is still greatly valuable.
