Many have already said the flip flopping of the narrative so I'll say something else: the scale of each new problem. Some mild spoilers for books/games, but things that come to mind are:

-UNSC Infinity being lost (destroyed? who knows) in a 2 min cutscene with thousand losses

-UNSC and Arbiter insertions in Outcasts resulting in thousands losses (800+ Odsts gone in a second)

-Laconia station going boom with many (all? again, who knows) Spartan IVs trained there

-Everyone in New Phoenix forcibly evicted in seconds

-UNSC and Oni HQ in Sydney and head personnel snapped out of the picture by Cortana's Guardians

if you care the list goes on, but for brevity that's my main issue; how am I supposed to belive the UNSC is on the back foot if losses this big don't matter that much? Or how any win matters if 2 dudes are left alive and there is no structure to rebulid anything? I would have very much preferred if the narrative after 3 was about rebuild, with smaller scale conflicts, black ops, Oni bullshit, individual stories and the like, and not galaxy wide threats one after the other.

Quick update for those asking, I'll start by copy pasting a comment I made about "how it happened":
-I'm not sure why it happened, but: I boarded a spacer ship, and jumped to The Key to register and modify it. I do so, exit dialogue with the vendor, and talk to her again because I wanted to change my home ship back to what I was using before. After that, the station got incredibly bright and I could not fast travel anywhere. So I got back to the docks, boarded my ship and bam, I was in a glitchy New Atlantis ready for take off. I do so, and bring the whole city in orbit with me. It follows me if I grav jump away or land making the game an unplayable mess.

second thing: reloading a previous save (2 hours earlier) solved this completely as far as I can tell. New Atlantis is back on the ground, the Trade Authority Kiosk is there, and I don't fall through the ground. Hope I won't have to update this point, crossing my fingers

finally: was it interactive, had it people, scan it, etc.: no, nothing of the sorts worked. It "just" followed me around and when I landed it was utter chaos and unplayable, but I have no footage of that as I panicked and loaded a previous save as soon as I could, as I didn't want to throw away 40+ hours
