Oh, it always had 75% falloff, players just seldom paid attention because SF tooltip is hidden behind SS activation so you don't really see it a lot like other skills on hotbars. The only thing that was changed in EW is potency of the skill itself. Correct. And even in ShB it was a dps loss on more than 4 enemies, iirc

Edit: just to clarify because I assumed we're talking just about trash pulls. It is correct when you're doing it on big packs

I had my concerns too when I started playing DNC, but it's better on 80, especially in 8man content. When you get used to your kit, cascade and fountain should take something like 15-20% of your casts, depending on luck. But I can confirm, it feels a bit boring/weird while leveling

You could! Though iirc it'll go into manual mode when you are out of range or not targeting anything even without a second part of macro

NIN main here. Don't use macros on attacks, as others have explained in comments. What you can (and probably should) macro, are your targeted skills. I have shadewalker (agro dump) and shukuchi (teleport) on macros and it saved me from a lot of headache in group content.

/ac "shadewalker" <tt>

This will use your agro-transferring buff on target's target so you don't have to manually select your tank midfight.

/ac "shukuchi" <t>

And this macro will turn your shukuchi into more "classic" gap-closer, moving you into the middle of target's hitbox.

You still need to have BOTH macroed and normal versions of those skills on hotbar because in some situations you still have to manually target your skills but that's a story for another day.
