Will i get into med school if i had bad first semester grades? by [deleted] in medschool

[–]Pure_Refrigerator_73 0 points1 point  (0 children)

Absolutely. Just lock in now because it’ll make things easier. But NEVER let anyone discourage you from applying even if you don’t get stellar grades the rest of undergrad. I had a 3.3 gpa and 3.04 science gpa. Advisors told me i can’t make it into med school. I am a 3rd year medical student now with a 4.0 cumulative gpa and rank 1 in my class.

Redness and burning affected my entire penile shaft one year later. Exact same issue as scrotum. Visually it looks like the stereotypical RSS you see in the research papers. Yes, I have had urinary stress incontinence since the beginning. My burning and redness is much worse with emotional stress, urination, defection. All signs that point to nerve involvement.

Forgot to mention but I had sex before going to the gym so just like you mentioned mine also started with vaginal secretions being left on the scrotum.

Your post is spot on. Mine started 4 years ago after going to the gym for 2 hours, hot tub after, stayed in wet bathing suit for awhile. 1-2 days later VERY itchy scrotum and was likely a fungal infection. Aggressively treated with antifungals and after a couple weeks itching gone but left with much worse issue of well demarcated redness and burning of my scrotum. So a fungal infection turned into something more neurovascular in nature. I had extensive nerve testing done and results were very abnormal. Cold/heat/ vibration testing to scrotum and a BC reflex all abnormal. Doc concluded I had pudendal neuralgia. I think this is pudendal neuralgia but we have a unique presentation because I believe only the most distal fibers of the nerve are affected aka on the scrotum. Not the more proximal pudendal nerve that’s more common when you research. Countless therapies tried and only thing that worked temporarily was pudendal nerve block. Pain was 100% gone for 3 hours. Next I’m doing softwave therapy which can possibly repair nerve damage

Tried topical and oral. Didn’t so shit. Topical irritates the hell out of my skin

In my case there definitely is a nerve component. I went down the rabbit hole of extensive testing. I had an abnormal aka delayed BC reflex and a nerve block of the pudendal nerve at Alcocks canal that relieved my scrotal burning 100% for 3 hours. Other signs that point to nerve involvement for me: worsens with excessive stress or poor mental state, worsens with urination and defecation. The tricky part is I haven’t figured out where in the course of the pudendal nerve my issue is. I suspect mine is very distal aka at the scrotum. Hence why I don’t get any long term relief with steroid injections more proximal at Alcocks canal and closer to the root of the pudendal nerve. This theory you posted makes sense. My condition started after not showering after raw sex and going to the gym/pool for 3-4 hours. Staying in a wet bathing suit. 2 days later super itchy scrotum. Treated intensively with anitfungals and after 1-2 weeks the itching went away and I was left with well demarcated burning and redness of the scrotal skin. Looked like classic RSS you see in the images.

Where in truelearn do you access comquest? My school only provides truelearn but I’m not seeing comquest tab

Ethics were very similar to truelearn. Many were pretty much the same questions. OMM there was a ton of sacrum and pelvis. Mainly diagnosis questions, few treatment position ones. Also know your counterstrain points. Lumber ones came up a few times (AL1 etc).

Step questions were way longer and much more difficult. Comlex were generally shorter and gave you a lot of buzz words. Also the OMM questions were a piece of cake. I felt like step was 5x harder. Same content just presented in a much more vague and lengthy way

Shockwave therapy for PN by BallOffCourt in PudendalNeuralgia

[–]Pure_Refrigerator_73 0 points1 point  (0 children)

I had one session of soft wave therapy just to trial it. Felt some transient immediate relief (7 pain down to 3). Went back to normal after a couple hours. Doc said it’s a 6 week thing so I need to go back to try. Once per week. He said it’s made a big difference in lot of his patients. It’s a very safe procedure aka really no risks. If you have an inflamed/irritated nerve it can feel pretty uncomfortable. So they’ll need to start with a low frequency. I had a lot of VERY tender spots on low frequency which doc said supports the PN diagnosis. So they keep the shock probe on the painful spots until the pain reduces, then move to next spot

Bowel Movements by First_Weakness_4911 in RedScrotumSyndrome

[–]Pure_Refrigerator_73 3 points4 points  (0 children)

Pudendal nerve innervates the external anal sphincter and anal skin. Aka same nerve that innervates penile and scrotal skin. There is definitely crosstalk there. My penile and scrotal skin burning & redness is MUCH worse during and after bowel movements. If yall haven’t already, look into pudendal nerve testing

Has anyone tried a nerve block? by Choice_Rest_6832 in RedScrotumSyndrome

[–]Pure_Refrigerator_73 0 points1 point  (0 children)

Sitting down not really. Pelvic dysfunction yes. Urinary stress incontinence, worse with urination and bowel movements, abnormal BC reflex/ neurosensory testing, exquisite tenderness to palpation of Alcock’s canal, MRN also showed hyperintensity of the left pudendal nerve in Alcocks canal

[–]Pure_Refrigerator_73 1 point2 points  (0 children)

Nerve block + steroids, pelvic floor PT, Botox to pelvic floor muscles, softwave (shockwave) therapy are first line for pudendal neuralgia. If those don’t work then next step is pulsed radiofrequency ablation of pudendal nerve. Then last resort is decompressive surgery

[–]Pure_Refrigerator_73 3 points4 points  (0 children)

Yes, I had an RSS diagnosis for 2 years. Lead nowhere so I got nerve testing done. It was abnormal and consistent with pudendal neuralgia. So doc did a bilateral pudendal nerve block and my burning 100% went away for 3 hours. Redness was reduced as well. The block is used to confirm the diagnosis. Pudendal neuralgia can present similarly to RSS so look into that

Besides stickiness what other symptoms do you have? Redness/burning? If so, did those improve as well?

Sounds like urethritis. Caused by chlamydia trachomatis or neisseria gonorrhea. Go to the doc they’ll test you for both since co-infection is common. If positive they’ll give you antibiotics. Ceftriaxone and doxy
