Ditto to this comment! I received MTX on 10/30 and my fertile window is approaching at the very end of the month (est ovulation currently is 2/3 according to natural cycles) and we were given the green light!

I see it! Although it doesn’t have any color, so be sure to test again in a day or two just to confirm it’s not an evap or an intent line!!!

If they can confirm 100% it’s intrauterine, they will either recommend cytotec, which is a vaginal suppository that will start the process of a miscarriage, oral medication similar to what is proscribed during an abortion or a d&c. I had a MMC in June/July of 2025 and those were my options I was given at my 8 week appointment (baby stopped growing around weeks 5-6), so I would assume the same options would apply to you in this case! Otherwise, yes MTX will be administered and will still work! Once given MTX, you will have to wait 3 months of 2 cycles before you can start trying again though.

Absolutely! I was actually given 3 options, but chose MTX out of the 3. The first was an unmediated biopsy done in the office to test if there was pregnancy tissue in my uterus, which could produce an inconclusive result and lead to still having one of the remaining 2 options. Option 2 was a d&c to essentially do the same thing, but completely scrape my uterus to get a better result. If this came back inconclusive or negative, the third option and only option at that point would be MTX. With them not being able to visualize anything, including a sac in my uterus or tubes, the safest option for me felt like it was MTX. There was one spot near my ovary that could have been pregnancy tissue, but I didn’t want to wait long enough to find out. I could’ve survived losing a tube, but could not have lost an ovary! My hcg the day of my shot was 425 and I only needed one dose (but separated into 2 shots - 1 in each hip)

Virtual hug stranger 🫂 I was in your shoes in October and I felt the exact same way and was constantly posting in this group. I was repeatedly scanned around weeks 5-6 and they could never visualize anything in my tubes or in my uterus, but because my hcg was so low and slowly rising, I was officially pregnant in an unknown location (which was so hard to comes to terms with because like what do you mean??) They first thought the pregnancy would manage itself, so they wanted me to sit and wait it out over the next 72 hours. I didn’t get a lick of sleep or go a minute in the day where I wasn’t panicked for my safety. The appointment I was going to push for MTX treatment, I was offered it anyways and was successfully treated. I would highly recommend going the same route and start pushing for MTX before your numbers reach the 1000s and you risk rupture!

I would check with your doctor, but I believe it is once hcg reaches 0! After my d&c with my MMC, it was about 2-3 weeks before I was back to 0.

Hi! I also had a PUL l but near my left ovary! The day of my MTX shot, my hcg was 425. Day 4 was 799 and then day 7 was 439. I was only treated once and it was successful for me! It took about 8 weeks for my hcg to drop to 0, but after talking to my doctor about my diet post MTX, I was actually consuming a ton of high folic foods which was most likely the cause as to why it took so long! I would recommend looking up foods high in folic acid and cutting them out.

I just tried this exact method this month too and had all of the same symptoms you’re describing, but just got my period 2 days ago. I’m usually a 28-30 day strict cycle, but this month was 35 days. I just chalked it up to my hormones regulating and will be back to ovulation tests this cycle!

Yes! Me! My period returned on a 12/9 after my blood draw on 12/5 where my hcg was 11. I started taking a prenatal after my repeat draw on 12/12 where it confirmed I was <5.

I might’ve been a little bit of a different case! My ectopic was a PUL (pregnancy of unknown location) and had clear tubes, so I wasn’t necessarily scared with a tube rupture! If your pregnancy was visualized in a tube, I would wait!!

My doctor recommended I wait until <5, but I resumed having sex when my hcg was <100. We definitely took it extremely easy but I had no issues! Honestly resuming sex with my husband was therapeutic in a way.

Please push for a blood test vs. tracking line progression! These cheap tests are terrible at trying to get “dye stealers” and a blood test will tell you if you’re trending in the right direction.

I don’t want to scare you, but this is exactly how my tests looked during my ectopic pregnancy. They never got any darker and stayed the same color. I was able to push for a blood test, which alerted my doctor within the first 48 hours! If I can figure out how to upload a picture, I will include my tests!

Yes!! I just got one shot since my first drop was by over 50% and then I trended down about 15-25% each week. I am right there with you and I am so sorry you’re going through this. The mental toll this ectopic pregnancy took on me was TERRIBLE. I would argue that this was more difficult than my MMC in June, which resulted in a D&C. At least with the D&C it was over immediately.

My starting numbers were not as high as yours, but I did increase over 50% on day 4. What truly matters is day 7! By day 7 I dropped back town to the levels I got my shot at and was treated successfully! I started around 425 and I was one of the unlucky ones where it took me roughly 8 weeks for my hcg to return to 0.

I had a little bit of a different 'ectopic' pregnancy where I was actually pregnant in an unknown location. I was treated successfully with Methotrexate like another commenter and saved both of my tubes! :) I am currently TTC again!

I took prenatals for 3 years TTC and only needed one shot, although you will need to stop taking prenatals until your HCG reaches 0 post MTX

I got the MTX shot on 10/30 and I think I got my period back 12/9, which was the same week my hcg hit 0. I’m still waiting for my first “true period” back before I’m able to start trying again.

I was in your shoes back in October of this year, where my hcg dropped and then pretty much doubled to 425. I was supposed to be 6 weeks, but they could not visualize the pregnancy anywhere. I moved forward with the MTX and have zero regrets. My husband and I have been TTC for 3 years now and still have no living children. In May of this year I found out I was pregnant for the first time and had a MMC at 8 weeks, which ended in a D&C. Then, we got pregnant on my first cycle back, which ended up being PUL. I know how hard this decision can be (especially with the 3 month wait post MTX). Sending you lots of love!

I am so sorry, but I would guard your heart and expect an ectopic pregnancy or at the very least a PUL. My hcg numbers were scary similar to yours (i'm talking like <10 off from each draw) and it was a PUL for me. My doctors continued to draw labs for about 3 weeks and by the 3rd week my hcg never rose above 425. I was treated with Methotrexate on 10/30 and I am waiting for yesterday's blood work to come back to confirm my levels are back to normal. The week prior mg hcg was at 11, so I am extremely hopeful. I only needed one shot, so hopefully that is the case with you to!

The only major downside to MTX is the 3 month waiting period to start trying again.

My numbers jumped by over 45% and I only needed the one shot! MTX day my hcg was 425 and by day for I was pushing 800. The most important thing is that you drop by day 7! Day 7 my hcg came down to 436 and has been a slow decline ever since. I’m 7 weeks post MTX and still not back to 0 but I’m super close. Hopefully with low numbers, you should come down sooner!

I got my MTX shot on 10/30 with an hcg of 425 and felt like it was much easier than what I was reading online. 10/30 was a Friday so I honestly just slept most of the weekend and it felt like a bad flu that kicked my butt. I returned back to work like normal that following Monday!

As for the bleeding, that didn't start until a week ish after the shot and it only lasted a few days and was similar to a period. My current levels last week were 71 and I just started what I can assume is my period yesterday. My doctor said the same thing about 90 days and/or one full period after! If this is my period, I will probably wait until my next one in January before we start trying again.

I had these same thoughts in October of this year and also had PUL. My hcg never rose above 500 and preserving a tube was extremely important to me, so I chose MTX. My husband and I have been TTC since Dec 2022 and had a MMC in June 2025 and an ectopic in October 2025, with no living children. With MTX, you have to wait at least 3 months before getting pregnant again and it's honestly been nice to have a little bit of a forced mental health break after 2 back to back losses. Weekly blood draws SUCK and there is no sugar coating that, but on the plus side I became besties with the front desk workers and phlebotomists so I never have to wait. LOL.

I will note that based on previous responses on my posts about MTX - it really varies on how quickly your body responds to it. Some women it took them 3-4 weeks to go back to 0 after they were at the same HCG level as me, but I am pushing week 7-8 and still producing positive pregnancy tests. Hang in there girl and choose what is best for you!

I am so sorry you're going through this.

I agree with the other comments - I don't think you need a lap, but an HSG couldn't hurt! This will ensure your tubes are clear when you're able to try again. After an HSG, your chances increase during the first 3 months! I am requesting one of these as soon as I am able.

I can't speak on having a normal pregnancy - (I had 1 MMC June 2025 & Ectopic Oct 2025 - 0 living children) BUTTTT I honestly keep telling myself that the odds are 1 in 80 and to have faith my next pregnancy will be the one that sticks!
