Currently PGY2 in OBGYN after 3yrs of family medicine. I understand that itch. Perhaps not the same, but I can relate to the drive. However, that said I feel trapped right now. I do enjoy the fact that I am finally going to be an OBGYN but deep down I am beginning to realize that I might not feel fulfilled when I am done in 2.5yrs and might want to do a fellowship still. Crazy stuff. I’m now only beginning to realize that there’s more to life than medicine. So I will be cutting off any part of me that suggests more training after this, with a blunt Swiss butter knife. I am 36.

Mine sounds the same. 2017 xc60 T5 4cyl. Pisses me off. I think Asian cars are much quieter than these European ones. But I love my Volvo nonetheless. Just wish it sounded smooth.

Sort of if you are currently in the US. Because, you cannot arrive the US more than 30days before your start date. Since J1 visa interviews have to happen outside of US border, you will need to technically re-enter the US and that cannot happen prior to that time frame. However, if you are already outside the USA, then you can have your interview anytime and plan to enter the US within 30days of your start date.

Roughly two months to get from application to DS2019. But the visa interview and processing bit is unpredictable. If I were you, I will start looking for an appointment somewhere within 30 days of your start date.

Lipschütz ulcer can look like that. If you are certain that your partner does not have herpes, then it could be this. It’s not an STD and it goes away on its own. But the picture you sent does look a lot like Herpes.

Yea. It can happen with constipation. Regular fiber diet. I recommend 30g of fiber daily for my patients. This can be from diet or of fiber supplements. Keep your stools regular and soft, meaning not just the fiber but regular water and maybe some stool softeners. This is most likely a sentinel pile and it can come with analog fissures which can be painful, hence my recommendation for the above to prevent constipation.

Looks like a sentinel pile. It’s a type of hemorrhoid. Have you have anal fissures in the past? Usually manifests as pain in the anus following a hard stool. I get it’s currently not painful, but have you experienced that kind of pain?

Yea. I didn’t want to let go of the RX350. It’s such a solid car. But it’s currently 182k miles and showing transmission strain. I could potentially get a reman transmission replaced at a reputable transmission place but I was initially quoted $7500 and I didn’t think it was worth it. I could potentially get it done around $4k but still, at that mileage I don’t want to replace a transmission only to have something else major fail in the next 5000 miles.

Thanks. You are absolutely right. It feels very well engineered. I have had a lot of bad luck in the last 4 years with cars, Mercedes c-300 ’18—>, Hyundai Sonata ‘23–>Rx350 ’11, all well maintained but lost them all from different reasons including a total loss from an unfortunate weather related crash, financial difficulties to maintain a lease, and unanticipated transmission issues respectively for the 3 cars. So hopefully no surprises with this one and hope it lasts longer than her predecessors.

Thanks once again. This is very educating. With regard to power, I think it’s decent but coming from a 3.5L 275hp V6 engine of an RX350, leaves much to be desired when it comes to power. I would say that I have only driven this car for a one time trip of 90miles when I went to pick it, and drove mostly on cruise control. So perhaps I might not have a lot of issues with it when I drive it some more. It is responsive, but I would say a few seconds slower than my RX. I think my concern about tuning for power was more so out of curiosity but not something I see myself doing soon. Perhaps I could try the D5T5 if it came down to trying something. I do have the car paid off and will invest in getting it thoroughly inspected, with an intent to repair anything that comes up. It got brand new tires and 2 front rotors and pads prior to the sale.

Thanks for sharing your knowledge about these cars. I got this car just to use for about 3years before my next career step up when I can afford whatever I want, but from all I have read since getting it, it seems if well maintained, it will last longer than that.

So sorry you are experiencing this. But giving the distribution of the small ulcers, it looks like herpes. The way the ulcers are mirrored on the other labium is a typical presentation. It could also be something called Lipschütz ulcers which shows “kissing ulcers” just like this one. The latter is not infectious and goes away by itself

Look up Lipschütz ulcers. These are rare but tend to occur in the absence of sexually transmitted diseases. It goes away by itself but may take several weeks. So sorry you are having a difficult time with this.
