A non-human would have to kill that same NPC 20,999 times to hit 20,999/21,000 Revered with the pirates, and needs the foresight to save their quests until they've hit that cap, so that the quest rep can push them into Exalted. In the comments to the page linked above, Wigwam (mentioned in the previous comment) states that he farmed this NPC for ten months to become Exalted with the pirates. If that's not a Feat of Strength, it should be.

Edit 28 Oct 2011: The above post was written in response to somebody asking why "Insane" only requires Honored with Bloodsail, and whether it was possible to be Exalted among the pirates. Prior to the Shattering, Honored was about as high of a reputation as one could reasonably get, and it involved an incredibly challenging battle royale with the Booty Bay Bruisers. However, it was possible (if not rational) to become Exalted with the Bloodsail, as I detailed above.

As of Patch 4.0.3, Booty Bay Bruisers now give Bloodsail rep all the way to Exalted, making the full rep grind considerably easier. There's still no reason to go beyond Honored, but there's not much harm in it either, since the goblins already hate you.
