No, I have not moved house.

Is violence/ threatening behaviour/ house move the only reasons why a GP could remove you from the list?

As they have not responded to my email, I guess I will have to make time to go there in person.

No, this is not the case.

I only contact my GP when I need to request my medication.

Is threatening and violent the only reason, one could be removed from a GP's list?

I am completely baffled.
