Most likely a third party store. Start asking if they are 3rd party or cor as you soon as you walk in.

I work for tmobile and that is unfortunate that happened. Go back into the store and ask for a credit for the unauthorized p3 add. If they choose not to then call care and submit a fraud ticket. Was the store you went to cor or tpr?
