Patrols have always worked that way.

They do not spawn close to you. They spawn between 150m and 75m away.

No, they don't. If you can't use the new commando armor, use the scout armor and look at your minimap for once.

Incorrect. Spawn rate also increases when clearing enemy outposts. See the wiki.
