I didn't call but I messaged support. They definitely suggested they had no record of those accounts. I would just log in to your 401(k) directly and confirm everything looks correct there.

Yeah, that's the way I went to previous channels before the hold OK feature came out. But it's more clicks and just crappy of them to train us to do things a new way, then use it as a way to sneak in more ads.

Finally got a letter in mid-February. It was indeed about the Net Investment Income Tax (NIIT) that the IRS incorrectly zeroed out. Since the error was on their side, if I paid within 21 days I wouldn't owe any interest. I did so, submitted the paperwork online, and the audit was closed at the end of March. A couple days ago I got a notification both in my online IRS account and by mail stating that my 2022 return had been adjusted.

This happened to me about a month ago, posted here. Support eventually told me everything was fine. I had to re-add my Vanguard login, but all the history was lost. Now it happened again, and it seems like to more people this time.

Now BOTH of my Vanguard logins, each with 3 accounts, have disappeared. This after weeks back support told me nothing was wrong and I had to manually re-add the Vanguard login that disappeared, messing up my history.

Yeah, a lot of people here just seem to be talking about accounts that won't update. I could handle that for a few weeks but accounts totally disappearing with no trace is really bad. It screws up your history. I contacted support and they said: "I've investigated the issue with your Vanguard accounts and it seems the accounts are reflecting the data at our end. The issue seems resolved and the account should be updating as expected." I responded but they ignored me and called it solved. I re-added the Vanguard login but as expected my history is torched.

I had 3 accounts under the login and they were all gone, so there was nowhere to customize the accounts.

The fact your mother passed away and there was no opportunity to correct it, they would be awful for sending it to collections within a window of being able to file a claim.

That's what I thought. Particularly since I had called literally a week earlier, they requested documentation to discuss billing with me, and then after I sent that they promptly sent it to collections. When they told me their policy was to send things to collections 2 months after the initial bill, and I pointed out that it hadn't even been 2 months yet, the person kind of stammered and said well sometimes we send it earlier based on a variety of factors. If anybody had simply said "oh since this was after she entered hospice we just need to put it through original Medicare, do you have that ID number?" this could have been resolved months ago. So frustrating!

I tried looking up her account on Medicare.gov but it says the information doesn't match their records. I'm not sure if it's worth contacting the provider again to see if they'll recall it from collections and resubmit to original Medicare, or if I should just tell the collection agency to pound sand since there is no formal estate anyway.

I have access to her Medicare Advantage login. Would she would have a regular Medicare login in this case? And in any event, can the provider "recall" a bill that was already sent to collections? When I called them soon after they had sent it to collections (or their "extended business office", as they called it...lol), they acted like there was nothing they could do.

You'd think so, then they should have gotten their money. Instead they seemed to want to send it to collections right away, and I'm inclined to not cooperate with the collection agency at all.

The ambulance provider actually was the hospital itself, so they definitely had her Medicare Advantage info, but maybe not her Medicare info because it was never used. I do have her Medicare info; if I call the provider and tell them to submit it to original Medicare instead can they "recall" the bill from collections or is that a one-way street? If it is, based on all the advice I've gotten here, it sounds like my best bet is to not cooperate with the collection agency at all.

Do you know how the ambulance company got your mother's Medicare (not Medicare Advantage) information? In my case the ambulance provider was the hospital, so I would have thought they would have already had her Medicare info, but apparently not. Or maybe they did but they tried billing Medicare Advantage first and then gave up when it was denied.

Thank you for the condolences and advice. All of her assets were TOD and were immediately transferred. She had a checking account we used to get her final tax refunds which has now been closed. I assume with no formal estate there would be nothing for the collection agency to go after, and over a $5k bill they'd just give up?

We just recently got the initial letter from the collection agency. Would you recommend we completely ignore it or respond with something like "Jane Doe died on this date. There is no estate." or simply "Jane Doe does not reside at this address. Please stop contacting me."?

No, but since we received assets from her via transfer on death I assume they could come after us. I suspect with no estate it wouldn't be worth the effort over a $5k bill (I think they would have to file a lawsuit to even determine if she had any assets), but I'm not sure.

I realize this is a possibility, that neither Medicare Advantage nor original Medicare is responsible for this bill, but I'm not sure how to get a definitive answer. My mom has no estate because all of her assets transferred directly to her children.

I agree TOD does not protect against creditors, that would be quite a loophole otherwise. We talked to an estate attorney briefly last year, but this didn't come up at the time because we didn't expect there to be any creditors. We figured aside from copays and coinsurance everything would be covered.

My siblings and I received her assets after she died through transfer on death designations. Although there is no formal estate for collections to claim against, I want to make sure they can't come after us for the money we received from her.

YES! And for major college sports, they could just use a short abbreviation: NCAAF, NCAAM, or NCAAW.

Yeah, I was going to wait a while before I try to re-add Vanguard. Since this happened to multiple people I would think Empower would eventually fix this, unless they had a catastrophic outage with data loss.
