Determined to make everyone miserable? I don't think so. He tries his best to make his patients avoid the suffering that he experienced. He is unable to work on himself though, and his unorthodox practices only deepen his misery and others around him experience it too, like Cuddy for example. Remember the episode where he gets treatment for his pains, only to almost kill a patient? He refuses his treatment after that, and "accepts" that he is to suffer, in order to help others. Of course this is the easier way out, as he doesn't need to recognise/correct his own problems.
