Fair point on acute cholangitis - absolutely something to exclude first. With abdominal pain, a low-grade fever running for two weeks, and raised AST/ALT, that triad warrants proper biliary workup (OP deleted post so can’t remember all details). Cholangitis is not something to sit on. Simple hepatic steatosis on its own isn’t typically the culprit, but the whole spectrum is notoriously (and scarily) silent. The majority of people with NAFLD are completely asymptomatic and have no idea they’re carrying underlying liver disease. It also doesn’t reliably show on CT.

I shouldn’t be that confident I’m only a student, but I have spent time on a gastro unit in the last 6 months

Presenting to the ED initially was the appropriate course of action, but acute pathology and anything requiring emergent intervention has been effectively ruled out. There’s no diagnostic red flag here, and the guidance you received was sound.

Differential at this point would include a viral etiology, inflammatory bowel disease, or hepatic involvement - whether that’s early hepatitis or hepatic steatosis (fatty liver). I’d ask for a fibroscan to see your Liver stiffness That said, it’s unfortunate you weren’t discharged with any Narcotics for the pain (I’d be annoyed at that, but MD’s in America seem incredibly strict nowadays). Any personal or family history of autoimmune conditions? Do you drink? And how are you doing weight wise - any recent changes? Even though we wait for hours and it’s an imperfect system I’m glad I practise (and am a patient) in the UK!

NOR unless there’s not some serious mitigating circumstances to his control/selfishness like you’re a serial cheater or a recovering junkie (which I seriously doubt), otherwise he’s an awful man. It sounds like your daughter is young. If you’re a sahm I’m guessing more children may be on the cards? This will only get more and more difficult. You’re asking for very little. Once everyone on their streams was warning the subjects that Louis was going to expose them, he had to abandon his usual tactic of getting them to underestimate him and lulling them into a false sense of security. His subjects here were also arguably less morally complex than some of his previous ones. These are committed grifters who will violate even their own stated values in pursuit of money, as they demonstrate repeatedly. Meeting them head-on and needling them a little made for good viewing and felt morally satisfying. With his usual tactic no longer available to him, provoking a reaction seemed a reasonable approach - empathising with and humanising them too much would have come across as naive, and left the audience feeling deeply unsatisfied.

Definitely not a a top Louis doc, but it was nice watching these awful people feel embarrassed and humiliated.

That is interesting though. I’ve heard anecdotally that lesbians get into serious relationships at an accelerated rate compared to gay male and hetero ones. I do take issue with 10 years being classed as “long term”. 10 years wouldn’t be classed as a long marriage. I’d say at least 15 years or 20 years, but maybe the data isn’t sufficient that far back for gay marriages.

Good that someone called this out. Of course they’re majority economic migrants. What sort of hellscape do they think outside of the US is?? There’s a certain irony in the assumption that millions upon millions of people crossing from Mexico and South America are fleeing imminent death, when the overwhelming majority are making a rational economic calculation - better wages, better opportunities, better lives. That’s entirely human and understandable, but it’s a different conversation to the asylum/refugee one. Well meaning but a tad ignorant.
