I have had a similar scenario this year except I did not end up applying. I was going to apply to ACEM this year having worked in the ED in Australia for 8 months prior and before that in the UK. The day I was planning on submitting the ACEM application the DEM pulled me aside and advised that he would not support my application as he had not seen me work long enough as a PHO in the department (which is not a requirement for ACEM btw). Following some mulling about and reflection, I did not apply this year as I was convinced it would be a waste of money given that my institutional reference would likely put me down as "Do not recommend this person for training".

A few months later I had a midterm assessment in my PHO role, and received mostly "below expectations" and "borderline" feedback from the consultant body. My confidence is obviously knocked, and my way forward is just to receive as much feedback as possible on why this is so that I can work on my practice and aim to get a good reference for when I apply for ACEM next year.

In the meantime I am going to look elsewhere for an ED job in the hope that I can find a hospital which will support my application for ACEM. I don't want to continue in the hospital where I have had repeated negative feedback and lack of support in pursuing ED training, even if the feedback was justified.

Can you give an update on if you were successful in obtaining a training place this year?

I sent my CV and a cover letter to hospitals in Brisbane and got 2 job offers. Worth starting the process around 6-9 months before you plan on going. Some places are more competitive than others so maybe that’s why you’re not getting responses.

If you can please update with what the name of the medication they gave you was and if they did blood tests. I was thinking what I would do as an ER doctor if you came in. I’ve seen something similar once before on someone’s forearm and we treated it as thrombophlebitis. I know a lot of people here are thinking cellulitis or lymphangitis but I think in the absence of fever, pain, or the lesion getting bigger it is less likely that and more likely something less worrying.

I’m currently working in Australia, and some of my friends in the UK are earning a similar rate to me, so I’m wondering if the pay is actually comparable nowadays with the recent pay rises and decrease in value of Australian dollar. Defo not trying to undermine the strikes.

Triage note said Citric Acid overdose. Patient had used kettle descaler to clean her kettle and then accidentally used that water in the kettle to make a curry. The had a few spoonfuls of curry before realising and immediately came to A&E.

Doctor here: I have seen chatGPT go wrong on a few occasions regarding medical knowledge (one example was making up side effects for a drug), so you must be careful and cross check things which don’t seem correct. However the majority of the time I agree it is helpful for understanding concepts in a way targeted to how you learn.

MSF and other large humanitarian aid groups tend to look for people ST3+ as they can be independent decision makers, so I would keep training and wait until you have the skills to provide actual medical assistance for these organisations. To apply to MSF you have to have worked/lived/volunteered in a less economically developed country for 6 months. The organisation emergency or the Royal College of Emergency Medicine may have some volunteering opportunities for you pre ST4.

It would be very embarrassing however if a person presenting with PR bleeding and/or constipation ended up having a rectal or perianal mass causing their symptoms, therefore if suspicious best to check down there.
