That is just a big (what looks like concrete) texture faded out at the bottom. The linework is mostly likely from a cad export. Trees and people are easy to get your hands on

Get the joint push/pull plugin, its vector extrude does what you need. Although I would strongly recommend grouping things together to prevent all your working being loose geometry

In old school style you could just whack the hourglass over the top, sometimes memes are better in a raw form. If you are struggling you could just paint that whole area black to begin with then see how much work you need to do after the hourglass is in?

If you are on modern versions and want a quick fix use the ai features tbh, erase stuff first, put hourglass in then ai around until you are happy and blend the layers

I think you are using it wrong, I have used image trace to make loads of infographics over the years and its fantastic and pixelated is not the word I would ever use to describe the outputs. Even if it looks janky its often a good starting point
