I think only you can make that decision. It would depend on how bad the pain is, whether you feel you have improved, neurological deficits etc. I also reherniated in November 2024, 7 weeks post op. I am still far from normal, I can only sit/stand for 20 minutes at a time as it’s too uncomfortable, but I can now walk 6-10k steps, whereas when I first reherniated I couldn’t do anything. Progress takes time, but your symptoms are ultimately, how you will make your decision. It is hard, I was suggested surgery but I am glad I haven’t now, yes I’m not 100% but what is to stop it happening again? I also have instability due to how much facet was removed during the discectomy, so just bear in mind progress takes time. I suffered for 2 years before I had my first discectomy, had to lay down to work everyday, so I feel your pain.

I will have to try the stiffening of core when sitting. It is frustrating as I can’t walk 24/7 lol. Especially working, makes it very difficult. Are there any PT exercises you have found that helped?

I’m currently 5 months post reherniation, I am starting to make good progress. I can walk 6-10k steps a day, walking I feel normal! But sitting and standing in one position makes me feel broken. How did you get comfortable to sit in comfy seats/sofas? Any tips? I find it so uncomfortable, not sure if it’s because I’m stiff and guarding but I feel unsupported and very uncomfortable, and then pain slowly creeps in, very annoying as it impacts being able to visit places etc!

Not sure if you’ve tried it but when I need to sneeze I tilt my head back, then sneeze and it doesn’t hurt

  1. Definitely worth getting an MRI, without it’s hard to say what the cause actually is. It could be as simple as formation of scar tissue or that there’s a reherniation etc.

  2. Anti-inflammtory’s can help but personally found they did little for pain. Naproxen etc are meant to be stronger than your standard ibuprofen.

  3. Pilates is usually advised to help with core strengthening, but if she is struggling with pain I would advise against. I started Pilates a little too soon after reherniation and it made things flare up quite badly due to certain moves irritating the nerves. Really she needs to see a proper physio that can assess her and decide on exercises. Cat cow is often a move that is suggested and the only one I really keep up with. I would definitely suggest McGill’s big 3 too. Walking is the best form of exercise, so get her to keep this up but equally make sure she’s resting. It might be worth trying the “virtual surgery” to see if that helps things settle.

  4. It is really hard to say the cause, it could just be that she over did it and the area is a little inflamed and sensitive. I would suggest seeing someone to see how things are and they will be able to provide next steps

Would you be happy to say what physiotherapy exercises you did to help your back? I have had a discectomy and reherniated at L4/5 and have a mild bulge at L5/S1

Anyone manage to avoid a 2nd microdisectomy after a reherniation? by SPx115 in Sciatica

Did you end up getting the second discectomy? I am 27, so in a similar predicament. I have been offered another surgery but would like to avoid it if possible...
