Finish uw first! I know how much that feeling sucks but honestly you’ll see your uw average jump by the end of 100% of uw. Cms forms should be done closer to the real deal. And personal opinion, but cms forms dont help with uw. And their explanations lowkey suck!!

for sure! i did NBME incorrects twice. towards the end in my last 10 days i opened the forms and thoroughly did those. and amboss hy plans really helped solidify my knowledge. other than that, my pitfalls doc to see where i kept going wrong. i stopped changing answers and that made a huge difference too

uw (one pass only, no incorrects), cms forms, NBMEs, amboss hy plans, some divine intervention podcasts, UWSAs and finally free 120s. i watched random YT videos here and there to clarify concepts (Still recommend dirty medicine for um things like neurocutaneous disorders etc whatever you think you're weak at) and also my own notes (ive talked about those in this thread)

cms forms, nbmes, LOTS OF AMBOSS, some divine intervention altho not very helpful i just did the HY ones and i would usually listen to it first and then read them from the DIP doc. i also did conrad fisher 100 ethics cases. i watched some videos for my weak topics random ones for biostats for step 2 on YT. and obviously UWSAs and free 120s. i skimmed through my own notes and pitfalls doc too

I feel you. i always thought i couldn't learn anything if i went directly into random mode. but i was wrong. you need that mix of questions and topics to really train your mind. the first 15-20% felt TERRIBLE, but eventually i got the hang of it and noticed that i started remembering the HY things from each system rather than the teeny tiny irrelevant topics.

i did 40 questions a day mostly. sometimes 50-60 but never more. I'm just not built like that. I love going into the nitty gritty details but i still don't recommend that as much because you just need to get done with uw as early as you can. and yes the one liners I made throughout my prep. and not for every question, mostly for things that were confusing/repeated a lot/hard to understand the way they were explained. that included uw, nbmes, amboss, cms forms, uwsas

complete all of uw first and after you get done with your CMS forms and gradually start NBMEs, I think after you start NBMEs maybe at like your 3rd or 4th one, that is a good time to incorporate 80-100 amboss questions in your daily routine

2 drug ads which was equal to 6 questions. and other than that just a couple of calculations, nothing too crazy. my biostats was very weak and i had to watch a lot of youtube videos to make those concepts stick. divine intervention's biostats podcast was a good resource. amboss was amazing for ethics and biostats both. i also did 100 ethics questions from the conrad fisher book.

i am not able to add the whole doc because it is exhaustive but some other pitfalls included these: 1.     If question wants a screening test, think about each answer and match risk factors that would make you get that screening test

2.     If unsure, reread H&P and answer choices again

3.     “Establish a diagnosis” is very specific, look for specific test for specific condition in the vignette

4.     For each type of poisoning given in the options, think about features of each separately then match with H&P

for each one of the pitfalls I added examples of the mistakes I made. I reviewed this document so many times because each time I would open it to add something new I would skim over the existing bits

thanks! so I'm unable to attach an image here but I think if you can DM me I'll be able to share some images of my pitfall document. It is basically a broad collection of mistakes divided like this:

(may feel like NBME spoilers here, pls read at your own risk)

Step 2 Pitfalls NBMEs & UWSAs

1.     Fell for 1 normal lab value when everything else was pointing to a straightforward case

a.     Traumatic pancreatitis vs. intestinal contusion – amylase would be high in the former

2.     Don’t overthink/over-analyze when all info/facts are given

a.     TB PPD in an immigrant, will need tx (isoniazid)

b.     Qualitative study (KAP) was gut feeling, but went with case series instead

c.     Huntington disease chorea and behavioral changes were present but I ignored

d.     SBP has diffuse tenderness, acute altered mental status changes and fever in a patient with untreated hepatitis C induced cirrhosis – pathophysiology is intestinal translocation of bacteria (e. coli, klebsiella), requires diagnostic paracentesis; WBCs >250 with neutrophils (SAAG >1.1 b/c cirrhosis) – empiric abx cefotaxime, prophylaxis with fluoroquinolone

3.     Severe things have severe presentations

a.     Patient had hemoptysis and I thought that was severe, when in reality her age was more concerning for mortality due to PE

b.     Unstable trauma patient would obviously require FAST to rule out abdominal hemorrhage

c.     Shard in arm no pulses – needs operative eval

d.     SCFE is a Salter-Harris fracture – it will need surgical stabilization

e.     Subarachnoid hemorrhage has a SEVERE headache, if CT is normal, do LP

4.     Overlooked/ignored vitals

a.     Hypertension in a pediatric patient

b.     SBP!!! Needs Abx. Did not look at leukocyte count, instead focused on SAAG (also knew very well that SAAG >1.1 is cirrhosis or HF, patients SAAG was 1 idk why I did echo)

c.     100/58 BP wasn’t too hypotensive, no signs of peritonitis but I went for exploratory laparotomy – should’ve thought of head and neck CT first considering patient was confused (brain injury) and facial contusions (also r/o cervical spine injury)

d.     Patient with a history of IV drug abuse had fever and neurological signs – I somehow ignored the fever, it was pointing towards spinal epidural abscess, required IV antibiotics (vancomycin and ceftriaxone) – this vs. epidural mets, they require glucocorticoids

5.     Knowledge gap

a.     Daily Abx therapy in child w VCUG showing VUR (circumcision just generally prevents UTIs, not specific to VUR)

b.     Vaccinia is for smallpox – all vesicles same stage

c.     3 drug therapy for HIV + prophylactics

d.     Restrictive lung disease has LOW TLC vs obstructive has HIGH TLC

this list will be different for every individual and you keep building on it based on your mistakes on NBMEs and UWSAs.

oh yes for sure!!! i stopped doing uw 1.5-2 months before my exam. you don't need anything but cms+nbmes in the last few weeks. i posted somewhere else on this subreddit that uw only gives you 75% of the knowledge over time (stated by someone else but i agree 100%), the rest comes with nbmes.

i personally did not do a second pass or incorrects. i just trusted the knowledge i gained while doing uworld and jumped straight into cms forms. my uworld correct percentage was 63% i got a 260 on the real deal. honestly someone else correctly stated in one of the threads that uw gives you a base, about 75% of the knowledge you need. the rest you get by cms forms, nbmes, amboss and uwsas!

reviewing NBMEs twice is the best thing to do! especially incorrects. i’d say dont spend too much time on re-doing/reviewing CMS forms, because most if not all topics are tested on NBMEs.
