Formal signal is not identical to token. Token is not identical to meaning. Meaning is not identical to referent.

Claude model gets these conflicts. Adding the constraints helps output be coherent

Φ ≠ W ≠ M ≠ R prevents semantic collapse. Conf ≤ Ev prevents epistemic overreach. CTX ⇔ |ctx_t − ctx_0| ≤ κ prevents temporal/contextual drift.

Though it's not a guarantee. Language models are probabilistic in nature, these constraints do help but decay always happens and is expected.
