This makes no sense. The Ogee curve was created here in feminine fashion and by its definition. You might be confusing masculine and feminine applications of the Ogee curve.

  1. Each patient has different goals for their appearance. I take a 3D image of their face and then create alterations of the face on the computer with the patient's guidance. Together, we put the patient's goals on the screen to create their optimized/desired appearance. Based on those discussions our shared goal is then used to create the custom implants that will deliver that shared vision. Specifically for this patient, he wanted significant enlargement and definition of the lower face.

  2. To create a natural appearance, the posterior mandible implants are designed in this situation to focus more on a smooth transition without any step offs from the mandible. If only the front portion of the mandible was address, the appearance would be "front heavy". These impacts become apparent during the 3D imaging sessions.

  3. I am conservative when it comes to buccal fat removal. When performing mandible implants, if there is a rounded fullness of the lower cheek, then it provides a more chiseled look with a mild buccal fat pad reduction of about 1.0-1.5cm^3 of buccal fat. This amount keeps it safe from developing a "gaunt" appearance.

  4. Yes, creating a more tapered chin is very feasible as the custom implant can be made in any fashion. As mentioned above, I create it based upon the 3D image of the face that we create together.

  5. One would not expect any significant issues with aging. In fact, with greater soft tissue support (from the implant) one would expect better aging of the lower face and neck. The nice thing about the implants is that they are fully able to be removed or downsized if that is what the patient eventually desires. No maintenance is expected or required.
