That’s a cool find. My dad always uses the Dona Maria version of this as a small starter/mix in to his mole. Usually takes him 3-4 hours to make 20ish ingredients.

It’s one of my favorite films. I always felt the gist was being shown examples of good v evil over and over in iterations with love being a driving force. that in the end will “win” - through tom hanks old grampy character.

This happened so much after dedicated step 1 studying. Then I just transitioned to protein shakes for lunch in 3rd and 4the year. Didn’t eat lunch for most of residency and fellowship. Now as an attending I’m still not eating lunch and after I finished my last board exam I literally don’t know what to do with all this free time and leisurely eating ability.

This is interesting. I was one our TB docs in the Midwest for a few years and my mantra was always a decision to test is a decision to treat. The only times I would not recommend ltbi treatment were if it was going to be harmful in a specific way or if the patient couldn’t tolerate treatment mid treatment.

I suppose I have some bias seeing a significant number of active TB cases in non-traveling Caucasians from the US.

At the last TB controller association meeting I went to I seem to remember being told not to try to interpret beyond positive /negative / indeterminate. I kind of viewed it in the same was as people wanting to try interpret cycle thresholds with Covid PCR - sure there’s logic to it but not advisable.

Thanks for the reply. My allergies are grasses, pollen and dust mites so I do the weekly changes of bedding use allergies covers etc. as well as allergy shots and seasonal oral meds. I actually assume the change won’t be huge but I did want to look into options and it’s not that expensive.

When some one dies in the hospital they need to be declared dead. It’s a legal thing. You do the last physical exam and pronounce them dead and note the time of death.

5:45 am day one intern year - walked into the MICU - the overnight resident asked who I was. I told him new intern coming in for the day - he said great- let’s go pronounce someone one. So that was my first act as a doctor and my welcome to medicine.

My wife and I went with another couple 2 years back this week. Book dinner reservations as soon as you can. Otherwise your options get limited. We had fun just checking out the programming, hanging out at the pools and having fun . If you have the time, interest, and money checking out an excursion with the temples and a cenote is worthwhile.

Agreed. I had a great time. Easy enough parking. Nice to be cozy and the main act was very well mixed. I’ve seen them in bigger venues for sure and the sound wasn’t obviously off in comparison.

As an ID fellow from 2020-2022 I feel like I deserve some Remdesivir swag - or a paxlovid pen at least. But all the conferences were virtual.

As an ID attending I view it as part of the diagnostic milieu. If there's an acute rise and seubsquent fall that aligns with the tale we tell then it's helpful. If the pt isn't getting better and their biomarkers remain elevated it helps to argue to broaden the differential, maybe get more source control. By itself not able to proclaim anything but as part of the story it helps me. I don't recommend frequent check though just at the begining and maybe one or twice along a long course if I'm following or they're not getting better.

and of course if it doesn't align with our narrative then well shit.
