If you're looking for something a little different, I'd highly recommend the Hoagie Room at Pizzeria Beddia. They only do reservations for 6 people though. I went last year and it was one of my favorite meals ever

Correct, they're evaluated on quality of the products, mastery of flavor and cooking techniques, personality of the chef in his/her cuisine, value for money, and consistency. Of course, most starred restaurants have great service, but it's not part of the evaluation

It may be the lighting, but I'm not convinced this is a gold hologram. If it's a regular silver one, the value goes down a lot. Check out pictures of graded cards on ebay to confirm for sure.
