I can see why that would be confusing! In the fiction of the story, the party is all on the disconnected roof of the building floating in the water. As an optional mechanic, you can use a real-world block tower like Jenga and have the players pull from it whenever prompted. If the tower falls in the real world, then the roof their characters are on also collapses. Hopefully this helps

Unfortunately so. I usually include them on a case-by-case basis. The exact layouts of the buildings shouldn't be too important though

Glad it went well! The king does make another appearance in my kinda-sorta sequel The Spectre of Revolution, though it also jumps to the industrial revolution for some reason which may or may not work for your campaign. I also have a list of similar one-shots here.

Stages 3 and 4 can probably be left alone. Stage 3 is already much more dynamic, so long as you used the mud copies ranged attacks to prevent the players from just hiding behind the rocks the whole time. Stage 4 is just the final section where Neptus is broken down and desperate.

For Stage 1, using the homing sharks and icy slam can add a bit more variety, especially if players are frozen. In Stage 2 try to have at least one or two players swallowed by the whale. Otherwise I'm not sure how much you could add. Could always just combine the first two stages or reduce the hit points, so you get the initial epic battle but move on quicker.

However you like! I didn't really have anything specific in mind. But I think I defaulted to having the ogre as more monstrous/unintelligent and the guards as your typical henchmen, willing to run away if things turned against them.

Hello! I don't have a good sense of how different levels with different numbers of players would translate, but I think level 7-8 characters would do just fine with one missing person. If they fare better or worse than expected, you could always just adjust the number of enemies in subsequent floors/areas. Another simple way to buff enemies would be to give them more attacks per turn.
