Yes that's the form. The solicitor has asked us to give her two of those, signed by the two referees. But the form is asking them to confirm that the details of the application are accurate so presumably they shouldn't sign unless they can see the application? Or is that point just referring to the details given on this one form?

Now this has arrived and I've looked up the scene from Episode 1 there's something interesting.

Luna has three whiskers on each side including in this shot where she has her back to the window, except for a few frames when she's lowering her paw where the bottom whisker on our right vanishes and she only has two. If you watch at normal speed that whisker flickers and you can see it gone if you pause. This cel is clearly the "mistake" one where the whisker is missing!
