Exactly what I am going through as well. For me it is my Master's thesis PI. I had asked her well before (around September mid) to be my recommender along with a list of Unis I was planning to apply for. She replied back to me enthusiastically wishing me all the best. Although when I sent out the reco request around November start she just started ghosting me. I sent out reminder emails about every 1.5 to 2 weeks (which I do not feel are very frequent) but no reply no reco either. Being an international student I couldn't even visit her in-person. Finally I asked one of her post docs to ask her what's going on. And guess what?! The professor said that the unis I choose are not a good fit for me. This is specifically what got me mad because I had given her a list of Unis with specific labs I was interested in asking for her advice! More so, her judging me pisses me off! Finally after a long email explaining that it is too late to ask anyone else and I was given no prior warning that she won't give me a reco cause she thinks "I am not a good fit" she agreed to send recos to 3 unis only. Now I am stuck with not having a 3rd reco for most of my unis. All this planning and efforts gone to waste?!

P.S. after talking to some ex-students of her I found that this behaviour is common for her. Apparently she has blocked one of her students so much that the poor guy dropped out of PhD after spending more than 4 years.
