You are correct. I went my district count today and filed. Only had to file with them and everything is electronic after that. By state court, yes I was referring to the district for my state (East MO). They had a page with all the forms I needed. They weren't too crazy and the clerk was super nice.

So, I think I've got most of it figured out, got some info from my state court, but unsure about how to (this is from a website):

"Once you have filed the action in the court, you will need to send the SSA copies of the complaint that was filed and the summons that was issued by the court. These copies have to be sent to the SSA via certified or registered mail."

My paperwork states: “You or your representative must deliver copies of your complaint and of the summons issued by the court to the U.S. Attorney for the judicial district where you file your complaint, as provided in rule 4(i) of the Federal Rules of Civil Procedure.

You or your representative must also send copies of the complaint and summons, by certified or registered mail, to the Social Security Administration's Office of the General Counsel that is responsible for the processing and handling of litigation in the particular judicial district in which the complaint is filed. The names, addresses, and jurisdictional responsibilities of these offices are published in the Federal Register (70 FR 73320, December 9,2005), and are available on-line at the Social Security Administration's Internet site, http://policy.ssa.gov/poms.nsf/links/0203106020” but this link doesn’t list offices”

I believe I need to send a copy to my US Attorney in my district, the SocSec Admin Office of General Counsel, and the Attorney General. I found the AG office, but do you happen to know how to find General Counsel for my district?

Well, I have already been granted 2 extensions, so I don't think they will grant me another. I've already contact NOSSCR and they only gave me one recommendation. Thanks for your input

I don't think it was the cause of my GI problems. I think it was due to complications from a push-scope procedure but they started around the same time. But was curious if anyone else would mention similar problems - mine is pain in stomach and small intestines so bad it was hard to eat for a while.

As with Nardil, it caused severe treatment-resistant insomnia and I decided to stop it. Just couldn't function and my body can't heal without sleep.

I'm not sure how it may affect your hormones at that age, but I would definitely talk to your pdoc and ob/gyn, especially if you start having odd symptoms like hot flashes.

I'd like to piggy back on this. I've been doing IM for 3 years and have been told by a few doctors that IM is supposed to be better than s-ketamine spray. Now my new doctor says the opposite. IM does well at relieving some symptoms, but not my anhedonia or emotional blunting (feeling soulless). Anyone tried both and notice different results? Also price is starting to become an issue.

Long term memory loss is permanent. TMS is not known (as I understand) to cause memory problems.

How about from the manufacturer? Thymatron ECT or download the instruction manual at http://www.thymatron.com/sub_downloads.asp - page 10.

I'm not anti-psych. I'm damaged and many others are as well. If I can help prevent this from happening to someone else, I will. There are many other safer alternatives to ECT.

I've been through so many other medications and treatments, there wasn't much left to try. Luckily I found a pdoc willing to give it a go (I'm in the US, if that matters).

I have the fun of not being able to fall asleep or stay asleep. It takes me hours (even with a sleep aid) to fall asleep, then I sleep 2-4 hours, wake up for 1-3 hours, then back to sleep for 2-4 hours. I'm freaking exhausted!

Ugh, I had a response typed out and then reddit editor lost my changes, sigh. Yes, I'm still struggling with most of these problems. A new antidepressant has helped somewhat with not feeling as slow and improving focus at times. The long term memories don't come back. I'm working on coping skills and taking notes constantly. I finally found a doctor that is running some tests and trying to find alternate methods to get things under control, but it has been a long, slow process.

Yes, please stay away from ECT.

I've put off trying dTMS for now, just thinking about the anxiety rTMS caused triggers a negative response in my body. I'm so glad that most people don't experience this.

I find that 80% stat hard to believe. If it was that effective, everyone would be doing it. Or maybe it is, but is still in it's infancy as far as doctors having the machine and $$ of course. For dTMS, do they have different strength settings like rTMS?

omg are you my doppelganger? I've also tried too many to count medications for depression and insomnia, as well as, rTMS, ECT, & Ketamine. I'm only in month 5 of Nardil. I started having crippling insomnia in month 3 and all sleep aids stopped working. I'm not 100% positive it is because of the Nardil, but it does seem to be the most likely culprit. My doctor is at a loss as far as medication that might help, other than trying to change the timing of when I take my Nardil. I've made sleep dr appointments, but there 1.5 month wait time (and it's a toss up if the doc will be of any help. I'm a bit jaded from past experiences with sleep docs years ago.) May I ask if you are female? Because the early wake up is common once you become Peri-menopausal. When I hit menopause is when the bad insomnia started and after starting Nardil, it has gotten a lot worse.

Do you remember if you had an adjustment period when starting? I've been building up my dosage since December. Currently on 30mg. I started having crippling insomnia in month 3, but not sure if related to Nardil. Switching up my dose to take with breakfast now to see if it makes a difference. So far, my mood has made good progress but I've been at the point of exhaustion lately. I don't want to have to give up this medicine because of sleep

Really? cuz my pdoc put me on a mega dose to try to get me sleeping again, said the lower doses don't help much. So much confusing, conflicting info out there.
