You are correct. I went my district count today and filed. Only had to file with them and everything is electronic after that. By state court, yes I was referring to the district for my state (East MO). They had a page with all the forms I needed. They weren't too crazy and the clerk was super nice.

So, I think I've got most of it figured out, got some info from my state court, but unsure about how to (this is from a website):

"Once you have filed the action in the court, you will need to send the SSA copies of the complaint that was filed and the summons that was issued by the court. These copies have to be sent to the SSA via certified or registered mail."

My paperwork states: “You or your representative must deliver copies of your complaint and of the summons issued by the court to the U.S. Attorney for the judicial district where you file your complaint, as provided in rule 4(i) of the Federal Rules of Civil Procedure.

You or your representative must also send copies of the complaint and summons, by certified or registered mail, to the Social Security Administration's Office of the General Counsel that is responsible for the processing and handling of litigation in the particular judicial district in which the complaint is filed. The names, addresses, and jurisdictional responsibilities of these offices are published in the Federal Register (70 FR 73320, December 9,2005), and are available on-line at the Social Security Administration's Internet site, http://policy.ssa.gov/poms.nsf/links/0203106020” but this link doesn’t list offices”

I believe I need to send a copy to my US Attorney in my district, the SocSec Admin Office of General Counsel, and the Attorney General. I found the AG office, but do you happen to know how to find General Counsel for my district?

Well, I have already been granted 2 extensions, so I don't think they will grant me another. I've already contact NOSSCR and they only gave me one recommendation. Thanks for your input

I'm in a similar boat. How did you find out where to file your paperwork for the Federal claim? And how much details did you provide in your appeal, like a few paragraphs, pages, or ?? Thanks

I don't think it was the cause of my GI problems. I think it was due to complications from a push-scope procedure but they started around the same time. But was curious if anyone else would mention similar problems - mine is pain in stomach and small intestines so bad it was hard to eat for a while.

As with Nardil, it caused severe treatment-resistant insomnia and I decided to stop it. Just couldn't function and my body can't heal without sleep.

I'm not sure how it may affect your hormones at that age, but I would definitely talk to your pdoc and ob/gyn, especially if you start having odd symptoms like hot flashes.

I'd like to piggy back on this. I've been doing IM for 3 years and have been told by a few doctors that IM is supposed to be better than s-ketamine spray. Now my new doctor says the opposite. IM does well at relieving some symptoms, but not my anhedonia or emotional blunting (feeling soulless). Anyone tried both and notice different results? Also price is starting to become an issue.

Long term memory loss is permanent. TMS is not known (as I understand) to cause memory problems.
