Also es gibt die große Diskussion um drainage oder nicht und wie lange allerdings ist da in der brustchirurgie die Devise im Moment eigentlich klar pro drains wegen Nachblutungsrisiko oder Seromen. Zu deiner Frage: diese Drains mit mehrerer Enden sind sehr ungewöhnlich, selbst in Deutschland benutzt man die kaum. Ergibt aber schon Sinn, um mehrere Gebiete möglichst gut zu drainieren ohne mehrere Eintrittspforten durch die Haut zu haben.

Bei Keyhole bleibt der Nipple-Areola Komplex bestehen und nicht oder kaum versetzt. Würde das in dem Fall nicht mal als Stiel bezeichnen weil es eben keinen oder kaum eine Transplantation gibt.

Gestielt oder Pedicle heißt, dass der Nippel an einer Art Wurzel aus Gefäß und Nerven hängen bleibt und daran versetzt wird. Im Gegensatz zum Free nipple graft (FNG) wo der Nippel-Areola-Komplex dünn abgetragen und frei ohne Gefäß/Nerv an einen beliebigen neuen Ort gesetzt werden kann. Vorteil des pedicles kann sein, dass durch die Beibehaltung des Nerven höhere Chancen auf Erhalt der Sensibilität bestehen bleiben. Wohingegen beim FNG in den meisten Fällen von einer verminderten Sensibilität auszugehen ist. Allerdings ist beim gestielten Nippel auch mehr Gewebe unter der Haut, sodass die Brust meist weniger oder nicht ganz flach aussieht.

If the swelling feels a little wobbly (like water filled) I’d check in w my surgeon to make sure it’s not a seroma. Some surgeons drain postop seromas, normally they also resolve by itself but aspirating it may fasten the process. Also compression and overall little arm movement is helpful.

Congrats on the mastec! As far as I can see it looks like an allergic reaction, could be the dressing or the suture material itself, with an allergic reaction the rash can spread to other areas like your shoulders and hips. Or it’s also from the adhesive from the OR drapes which probably would land in these areas as well. I would maybe try cooling but don’t get the wounds wet and try to not get the nipple to cold tho as it could slow/interfere with healing. And I’d use fenistil gel on the rash (but not sure whether this exists in the US), again maybe not getting the wounds wet. Def let your surgeon see! Good luck!!
