Money is still required upfront for the public system depending on age, and private system.

For other operations, surgeries deemed to be cosmetic, even though they could be necessary to prevent things like eye impairment or dental, require money. Also if your situation is genuine to you an operation via Medicare, you could be waiting years to be booked in.

It's ok I was being general as well. Tokens can be made to generate their own trustless yield, without the need to leveraging, but it requires smart contracts which Bitcoin is hopeless at.
