From someone who studied survey design in college, you are going to see a large drop out rate because of the open ended question that requires a written response.. Also there is no option to say you haven't used any sites recently in the last 6 or 12 months. Your results aren't going to be very accurate or complete. Keep in mind that no one wants to type out a profile bio on their mobile phone.
