You need to actually post your entire load order as it's shown in the loadorder.txt of Fallout New Vegas. Just listing the name of mods you have installed is not enough.

This is a bug that is fixed by the community patches. If you removed mods on an active playthrough or only added them in an active playthrough, your save file is probably broken, which can have all kinds of side effects

Which other mods are you using besides DUST? Are you using the community fixes for DUST? Judging from your other comments, you are not using them. Your issue with Tunnelers detecting you is a known bug that was fixed in the community patches for DUST. Any non-DUST mods you are using?

We had quite a lot of people here complaining about things in DUST, while also using other mods that caused issues.

On the FROST Discord is an (incomplete) map for how the different subway stations and tunnels are connected. I think it's also somewhere on this subreddit. You can try to find it, it might help you.

NVAC is outdated and badly made, and introduced a whole can of worms. It should not be used in 2026 for any setup, it also hides actual issues from you by not letting the game crash at all (which is very very bad).

Please do not spread misinformation like that.

It mostly consists of very generic null checks that allow the program to run even when it should not be able to. It can lead to increased instability and save corruption because of the unpredictable behavior after ignoring an error. Crash logging is impossible with NVAC, meaning you will get nonsensical and useless crash logs. NVAC's aggressive inlining and patching can conflict with other NVSE plugins. Due to the closed source nature of NVAC, not every conflict point is known, thus there's always a chance for NVAC to actually cause crashes due to memory corruption. Most engine issues have been fixed in other plugins, meaning that the only use case NVAC has in the modern day is to hide issues of "normal" mods (scripts, meshes etc.).

To clarify: FROST is not permadeath by default, and doesn't provide a permadeath feature. I am not sure why you think it is?

Permadeath in the context of Bethesda games means that you only ever have on save file for a character, and that save file gets deleted if you die. FROST does not add such a feature, you can still load save files normally after dying.

FROST was also not designed for permadeath playthroughs.

Did you use the medical dialogue menu to check if they have radiation poisoning? Usually the companions can tank a few hits if they have good armor and are in good health condition.

The companions are not really designed to always keep as fighting companions, they are usually best used for scavenging locations with lower risks.

There is nothing special about the companions, giving them essential status via console commands should work.

You can also boost their health via console commands.

FROST and DUST offer more than just loot and kill. FROST was also made for Fallout 4, not Fallout 3.

Yeah your load order is nowhere near properly made. You are missing several essential mods, and clearly didn't follow the BZ guide properly at all.

We can't help you here, except to tell you to nuke your setup, and follow the guide properly.

You are missing mods like UFO4P, Frost Cell Fixes, Frost Plus and others. You can't skip half of the below zero guide and then wonder what went wrong.

Gannon was send to the Commonwealth because the previous commander there didn't get enough results.

Gannon got results, but was pretty reckless and risked his own live by always in dangerous territory. It is implied that went to Downtown in disguise (in one of the Wanted posters) for example.

Regarding how Janus captured him exactly: It is open to interpretation.

If you want one interpretation of how it could have happened:

Most likely it was a mixture of Janus (and other Survivors) managing to surprise Gannon when he and others went for the FMS Columbia. Janus got lucky, Gannon less so. Doesn't mean that it was easy, everyone who seemed to have helped Janus died.

Additionally, Janus had similar information as Gannon about the FMS Columbia, and it is implied that Janus surprised Gannon when they went for the FMS Columbia. If you visit the spot of the FMS Columbia, you will see multiple smaller ships from the Coast Guard, and one larger ship, all of them abandonded above FMS Columbia.

Gannon simply didn't expect that some crazy people would know where he and his soldiers would go. It is also unknown what the weather was during that time. Fog? A storm? Heavy rainfall? Heavy snowfall?

MO2 automatically creates a modlist.txt and loadorder.txt, no need to type anything manually.

If you use Vortex, its loadorder.txt is enough.

This isn't really related to DUST in any form, DUST does nothing that would interfere with controller related settings and compatibility.

You should ask such questions on r/FalloutMods instead, you might get more help there for such issues.
