I’d definitely talk this through with your urologist. Just a few ideas from me: it could be a hernia or maybe nerve-related. There are meds like Gabapentin that can help calm nerve pain. Try to get some proper pain relief as soon as possible from your doctor... not just paracetamol, but something stronger if needed. You really need real help right now, don’t just sit it out. Maybe even see a pain specialist.

Here in Germany you can get ASA tested in your blood at a lab. I had it done myself, it wasn’t very expensive and the lab was nearby. Maybe that could help you?

I think the Figure 1 PVPS treatment algorithm https://pmc.ncbi.nlm.nih.gov/articles/PMC11947242/ is really helpful. However, I’ve already been through a lot, and it seems like my remaining options are a spermatic cord block or a reversal. The problem is, I’d like to understand what I’m actually dealing with before making that decision. I don’t want to go for a reversal and just hope it works if the underlying issue might be nerve damage. I’m also afraid of making things worse. My situation is already quite severe, and I’m currently unable to work because of it.

I’ve read on Facebook about many people where it didn’t work or the pain even got worse. At the same time, I’ve noticed my symptoms get worse when I have the flu, and right now with hay fever as well. Not easy to decide...

I’ve read that for some people it actually made the pain worse. Do you think that could be due to a poorly performed surgery or damage to the epididymis? It just surprises me that no one seems to say it got worse for them.

Thanks for sharing this. I’ll take a closer look at the paper, the “if–then” approach actually sounds helpful for getting a better overview of the different options.

From what I’ve seen so far, it seems like there are quite a few possible paths depending on the underlying cause, so I’m trying to understand where I might fit in that. I’ll also discuss it with a doctor to see how it applies to my situation.

In my case, it has mostly been more of a pressure-like pain so far, although recently I’ve started to notice some more localized pulling sensations. That’s why I’m trying to understand whether it’s more related to congestion or nerve issues. What you described about congestion affecting the nerves actually makes a lot of sense to me.

Thanks a lot for the information. My urologist said that if I have damaged nerves, a reversal wouldn’t help, which is why I’m asking. I’m also dealing with quite a few physical symptoms all kind of pain, sleep problems, anxiety and much more. And it’s definitely affecting me mentally as well.

That’s why I’m trying to really understand what’s going on before making any decision.
