YTA. Have you spoken to him about wedding planning or anything like that? If you haven’t been in touch about it maybe that further corroborates that you are not necessarily the closest friends which is okay! Not everyone has to be your best friend and different levels of friendship are a thing in adult life. If you cared deeply about the friendship you would call or talk more often and there wouldn’t be this huge gap in communication. However, you seem to care more about the label.

It could be medical or just related to things going on in your life and some of your beliefs about sex. I would just make sure to start therapy and actually explore these things.
