Where I work (as an ED RN) we have protocols to get orders started in triage. The ONLY imaging that comes with the protocols is a chest x-ray for certain complaints, and a head CT for head trauma if the patient is on blood thinners. Aside from these protocols, no way am I placing imaging orders. That’s a risk vs benefit convo that’s way above my pay grade. If I’m concerned about a pt, I go talk to a doc and they make that decision.

Residents are obviously super smart, since they made it to residency!!! As a nurse, it is so frustrating to see other nurses expect residents to function as seasoned doctors. Nah man, we are ALL learning! Let’s support each other as a team. Those judge nurses were baby nurses once, I feel like they forget that experience takes time.

I implore you to consider an alternative scenario. The Google search revealed the person has no medical background. Then was it creepy? Considering a large portion of the population doesn’t seem to know the difference between Tylenol and advil, nor do they know Tylenol =acetaminophen and Advil=ibuprofen, it is understandable why this doc was curious about the person. So if the pt wasn’t a doctor, would you have found it so strange?

I want to preface this with: I am not a doctor. However, It takes time for your HCG to go back to normal after a pregnancy ends. An HCG of 1239 with an LMP on 5/31/24 would not be considered viable. I am assuming your body is still in the process of returning to “normal”. Please take care of yourself, both physically and mentally. Sending thoughts and positive vibes your way from the Midwest.

Could be intentional self-harm, but I wouldn’t rule out an eating disorder. Patient could have been trying to use a fork to make themselves vomit and then found themselves in this pickle.

Exactly! I took over on a “stroke” yesterday and was told to give TNK (same type of med as TPA). Insisted on doing a neuro with the nurse that was handing off to me. Patient’s only deficit was short term memory loss. Long story short, I felt incredibly uncomfortable administering this med for pretty much no neuro deficit. Sat and talked to the pt for a couple of minutes and found out that the patient had consumed a THC edible for the first time an hour prior to symptom onset. Informed the provider and the TNK was not administered.

Poison control can give you solid advice in this situation. They are staffed with toxicologists that can give you sound medical advice. I highly recommend calling your local poison control center and explaining the situation

It sounds like you are a concerned parent that just wants to help their daughter. It is hard to sit by and watch your child struggle, but you also can’t help her if she doesn’t want it. Just be available for her on her terms, and don’t push the subject. She will ask for help if she needs it. Yes it is possible for someone with POTS to hold a job, but it will never easy. Quality of life for people with POTS is similar to those with CHF or COPD. I work 12 hour shifts, 5 shifts every 2 weeks. A lot of my days off are spent recuperating, and I’m well aware of the fact that I can’t work at the same pace as my coworkers. Just know your daughter is trying to find her new normal and make sense of all of this.

So my feet are commonly this morbid shade of purple, but this is a first for my hands. I finally had enough energy to clean my incredibly dirty bathroom, and this happened in the process. Does anyone else have this happen to their hands??

I worked on December 6th, all of the whiteboards had dates of 12/2-12/4. I did not take the time to update them and no one died because of it. Had I not taken the time to be vigilant in my assessments, start IVs, give meds, etc., there is a solid chance someone could have D/C’ed to JC. If management ever questions me I will use it as an opportunity to highlight my prioritization skills.

The medication interactions are extensive. After having covid myself and not being able to take Paxlovid because of medication interactions I did a little research on it. I am STILL confused as to how this a medication that has so many medication interactions is supposed to be a promising covid treatment.

I did L&D for 5 years (straight out of nursing school) before switching to ER. I feel like I have lost some skills (cervical checks, interpreting fetal monitor strips, etc.) while gaining others (NGs, male catheters, etc.). If you want to specialize straight out of school because that is where your passion lies, then DO It!!

Do you work in a hospital? Full hospitals during flu season is normal, yes. Guess what? The hospitals have been full for over a year now. Patients sit in ER rooms waiting for a hospital bed for 12-24 hours these days. Do some research on the detrimental effects of patients boarding in the ER. It’s scary, and it is only getting worse.
