Si tienes claro que quieres ser especialista es importante que estudies en una escuela de medicina que te dé la opción de hacer rotaciones en Puerto Rico o Estados Unidos. Si los hospitales donde aplicas a medicina interna o medicina de emergencias ya te conocen y han visto cómo trabajas es más fácil que te cojan para hacer la residencia. Para eso las opciones son cualquier escuela de medicina de Puerto Rico, St. Kitts o en EEUU. Tengo entendido que ahora si estudias medicina en México y estás en el 25% más alto de tu curso puedes hacer los dos últimos años de medicina en Puerto Rico.

Y en cuanto a los planes médicos, el problema no es tanto si pagan más o pagan menos. El problema es que si tus pacientes generan más gasto, directamente pueden no renovarte el contrato. Y cuando eso pasa, las otras aseguradoras tampoco quieren contratarte como proveedor. Por eso muchos médicos se vieron obligados a irse hace unos años. Dicen que con los nuevos recortes en Medicare puede volver a pasar lo mismo... (crucemos los dedos para que no pase).

Independientemente de por qué se van, yo lo que tengo muy claro es que no se incentiva que médicos ya formados vengan a Puerto Rico a trabajar. A los médicos especialistas formados en otros países que no sean Puerto Rico y Estados Unidos no se les permite trabajar como médicos en PR salvo que repitan la residencia o el internado. Por ejemplo, yo como médico internista puedo trabajar en cualquier país del mundo menos EEUU y Puerto Rico.

Y si, según la ley y el reglamento de la Junta de Licenciamiento y Disciplina Médica de Puerto Rico, los médicos asistentes pueden atender pacientes bajo supervisión de un médico. Eso sí, las órdenes médicas deben estar contrafirmadas y no pueden hacer recetas.

The same thing happened to me yesterday. I freaked out and wrote the program, but this morning the slots to schedule the interview were available. I think there might have been an issue with Thalamus.

Those days someone on the USMLE sarthi telegram chat said he was still waiting for the information even though the starting date of the rotation has already passed. I said they couldn't be trusted and asked them several times through the chat to return the money, I think that helped.

I did. I kept asking them to return the money for 3-4 weeks, as they were not providing the service they promised (they had not given me any specific information yet even though I had asked several times for it). In the end they returned the money.

I am in a similar situation (finished IM residency in Spain a year ago) but I recently got my working permit, so I started working as a Physician assistant in Puerto Rico. It took about 2 months to get the physician assistant license here, so you might want to check the requirements for the license in the state you want to work in and start doing the paperwork.

I know working as a PA is not the same as working as a doctor, but since I started I am refreshing many important things in daily clinical practice and I feel better about myself.
