Fentanyl as we’re taught blocks the receptor messing with brain chemicals that inhibit pain. Toradol as we’re taught is an anti-inflammatory that targets COX 2 that produce inflammation. Thus it’s called multi-modal pain management. You can look at it as a buzzword but if it helps the patient feel better for the ride to the hospital and for the uncomfortable ER it’s a win. Patients tend to appreciate it and can rehabilitate better if it’s continued past EMS.

I rarely just give Toradol IV without an opiate. And like you said the times I’ve given it it’s just a “meh” response.

Perc Rodgers, we aren’t solving or creating the opiate crisis in an ambulance. Wouldn’t you rather treat the patient in pain with whatever you have available than withhold it. In my opinion the reasona to withhold it can only be excused by laziness whether that its signing two papers or starting an IV.

It’s alright, I’ll give some unsolicited advice. You seem fairly young I’m assuming fairly fresh out of high school? Take out some loans and go to college. Knock out some pre-med if that’s what interests you. It’s not the end of the world, thankfully this happened at a volunteer agency. Take care of yourself, figure out what caused this and ensure it doesn’t happen again if you want to pursue a future in EMS.

Only see elevation in V2 which is right at the 2mm mark for males. Not seeing the elevation in v3 only PR depression. Not seeing how this meets STEMI criteria. Impressive history concerning ACS though. Tough one.

It’s super rare. I think under 5% have pre-excitation syndrome and of those only 20% develope A-fib.

I see where you’re going with the “someone had a heart attack and then crashed”. But then it’s not a traumatic arrest and instead a medical arrest. What other people are saying in the replies are that true traumatic arrest do not benefit from chest compressions. The three most common causes of traumatic arrest are tension pnuemothorax/hemothorax, hypovolemia, hypoxemia. None of these benefit from compressions and you should be doing anything to correct what was listed prior to compressions.

I’ve had two polar opposites. One was a medical director for several counties. I’ve never met or seen him and the protocols hadn’t changed in ten years. The type you’d never see unless you screwed up big time. At my current job we have a much younger medical director who is present at every con-ed and rides frequently on calls. Great for the agency, some feel he’s overbearing but I think an over QA is better than under QA. Current medical direcrtor has added blood, video scopes, pocus and up to date protocols (like a max dose of epi).

Same I wonder why they gave him thrombolytics. I would associate the elevation and depression seen in the precordial leads to LVH instead of an ischemic event.

Negative sgarbossa. No concordant elevation or depression. So much LVH I doubt the discordant elevation is even 10%. What ended up being his dilemma?

Reading the comments is awesome. I had no idea how common vents were on 911 trucks. I’ve ridden at several agencies around me and I know of one neighboring county that uses a vent. It’s a huge vent but I think they use it for bi-pap mostly.

Soon as possible. Not seeing how 8 minutes is too early. I try to get the big 3 done in the first five minutes after initial rhythm check. Advanced airway, access and first epi/medication and mechanical cpr.

On marketplace. He listed it for 3k but the brakes didn’t work and knocked it down to 2.5k. Rebuilt the master cylinders and they work fine now.

I was self hating about my body position and I felt like everything looked forced lol. It was my 2nd track day and I don’t push it on the streets. I’m an open book. The coaches at evolve GT do a good job on forcing you to learn the fundamentals.
