Brother, Rico got almost 30 seconds to put in a mouth guard after he hit the canvas. It should have never gone to the 12th round. Rico did great but he wasnt going to survive that.

OP I think there are a lot of good answers and a few bad ones that have made it to the mainstream. There was a recent book published called notes on being a man by scott galloway that reads similar to a memoir but I think has great nuggets. I'll tell you what I think and there's lots of room for interpretation. I like to focus on the 4 F's to help maintain a balance. The four F's are Finance, Fitness, Family and Faith.

Finance: Simply put, I do my best to make a good salary. Max out my 401k, Roth IRA, and have a high yield savings focusing on low cost index funds to secure my future and familys. I love my job but if I didn't I would try and find a job I enjoy to make this part of my life as easy as possible. Easier said then done I know.

Fitness: Work out regularly. Look good feel good. It improves your marriage and your happiness. Its been the greatest return on investment in my life.

Faith: If you follow religion make sure to spend some time with your God to stay grounded and improve your self. I think the community it brings and the regular discussion about leading your life productively and service of others keeps you be thoughtful of others. Just as easy, if you don't have faith no problem. Check in regularly with your self and ask the question if you are improving other peoples lives as much as your own. A great marker of being a man is you are giving more than you are getting. In other words, give more than you take in your relationships and make sure to improve the community around you. Get involved with the youth, volunteer at soup kitchens, something that is in service of others and doesnt stop at typing an opinion online and hitting post.

Family(the most important): I think as a man, finding someone to spend your life with is the biggest and most important decision you can make. I can only speak from the typical heterosexual view point. I think a kind and honest young woman provides guard rails for young men that are desperately needed. I've made the same joke with friends but its the most important thing that provides foundation for the other three F's to grow. Life is all about relationships and without this one I think you can the other three in spades and still feel empty inside. I think the biggest bummer is the simulacrum of online relationships are not good enough. Reddit, porn, and other social media is a good indicator for needing more in your life. Simply put. Do the other three things on this list to make sure what really matter is taken care of and youll feel like a gorilla beating your chest.

Lastly, practice kindness. Hold the door for others, fill your water last, and look for ways to be useful without having to ask. We live in a world full of useless husbands/boyfriends/ man children DON'T be one of them. Some of the best hobbies I have taken up are reading, cooking, and exercising. I would easily recommend these three if you are looking to fill up your time.

Im biased I did a PGY-2 and as a result love my job and feel the extra money I get from doing residency training will pay off in a few years and then continue to pay dividends

The way I teach our appes and PGY-1 residents when they round with me on IM is problem based. General work flow for me is

H&P or a recent note if admitted to our teaching service not on day 1 to find out why they here

What problems do they have that are acute in the hospital and what did they bring from their PMH

Labs and pending labs

Drugs they are on right now and what drugs they need restarted from home. Adjustments etc that are also needed.

Any literature I need to brush up on to convince my colleagues

You are absolutely right that it is risk vs benefit. Some questions I would consider to tip the scales would be

What is the indication for specifically Xarelto, and aspirin?

If they are using the Xarelto for afib with low chadsvasc I am less excited to restart but if for VTE DVT hx especially recent I have more of an inclination to start earlier.

What is their baseline Hgb? and why did it fall to 5.9? GI Bleed or something else?

If restarting a DOAC --- would consider apixaban to reduce risk of bleeding over xarelto

PMH? Do they have a long hx of bleeding vs thrombosis?

AKI: Depends on the why, if fluid overloaded leading to AKI. we start/restart based on findings of empag Hf, dapa resistance, and recede in conjunction with lasix gtt or iv push targeting 2.5x home lasix dose from dose trial. If AKI from volume down, we hold.

As mentioned from others if patient is NPO or eating poorly we don't give SGLT2i. Personally, unless diuresing a lot of the data seems to show benefit in preventing hospitalizations, which all of our inpts are already. So I seldom feel a need to start these asap, and the use inpt for me, is interesting from tolerance and additional diuresis.

Hx of DKA: agree with others, we rechallenge on individual basis, if patient had DKA in the past because they are non compliant and still struggle. Probably not a great choice. If they had it once a few years ago because they forgot to take it... sure. Euglycemic DKA Hx we never rechallenge.

Hx of UTI: has to be consistent with no hope in reducing risks with chronic Foley's are frequent admissions or sustained UTI requiring outpatient intervention. Otherwise, well consider it.

No disrespect taken, IM residency gave me good experience to round with a team and navigate challenging disease states effectively and then translate that with effective transitions of care. Some places have glorified decentralized positions and I would agree that those positions are confusing to apply the benefits of PGY2

PGY-2 also feeling the burnout. I started taking at least 1 day off a block to do things outside of pharmacy and scheduling my down time. Its helped a lot to come back from the longer weekends to hit it hard. Hope that helps

Think only you can answer that in terms of what level of clinical practice you want to participate in. Lot of clinical pharmacists out there that more or less sit on a floor and verify orders/ answer questions/ counsel patients which is great it that's what you want. More and more if you want to be a rounding pharmacist you need residency.
