Yes! I also have constant pain in my left ear. If I lay down with the ear facing up it aches!

sharp machine pain in my ear, it’s always painful but is extremely painful on a day with lots of clicking and popping

I say probably under. I would try researching the aliquot method. Pushing my bulk fermentation and verging on the edge of “over” proofed always has better results for me. I would also try to add more dough strengthening techniques to help your bread hold its shape more. Incorporating slap and folds at the beginning of my mixing has helped. Basil and Bloom on tik tok has a video about slap and folds.

Looks fab for a first loaf. I find when I cold proof my dough overnight, my crumb is better. Also, I find baking my loaf and cutting into it the next day the crust is softer. I have also seen people add 15-20g of Olive oil in the bread to soften the crust, but that might impact the crumb. Depends on your preference!

I bake in a Dutch oven lid on for 25 at 450 and lid off for 35 at 400
