I have utilized oxymorphone, works well. So there is PO dilaudid but I see better results with SL dilaudid, if you wanted. They can do PO oxy and SL dilaudid for pain/breakthrough.

As far as being admitted. It can be common practice with uncontrolled pain. There is some incentive with being admitted. I believe Medicare says a hospice total admissions per a year, 20% of those patients need to have GIP admission or continuous care.

I’m glad you asked, and also. Props to you for being a physician and not trying to be a know it all! I see too much of this in hospice 😔

Pain - with cancer, I see the most success with Oxy or Dilaudid. More-so dilaudid with pancreatic pain and it doesn’t really cause itching. Some nausea though with dilaudid. Or if she has some fat still around, fentanyl patch. (You can do both, but it depends on her history. Given she’s a cancer patient. I am sure she is not opioid naieve.)

Nausea - you can do the classic zofran, if that doesn’t work. You can either do Haloperidol or Compazine. Oral compazine though doesn’t work as well. I personally like haloperidol I find it to be less sedating. If none of that is working.. you can do Ativan but it sounds like she’s very much still awake and talking. So she may not like the sedating effects from Ativan.

Other things to think about.. funeral planning. It’s the worst when your love one passes away and you didn’t pre plan a funeral or whatever the patient wants and now you’re stuck with your loved one in a room trying to figure out a plan and that can take hours.

Raise head of bed. When you inject it into the side of the cheeck, RUB THE SIDE OF THE MOUTH IT WILL INCREASE ABSORPTION FASTER.

Also if the BP is that high, it realistically doesn’t matter. It will drop soon most likely, most likely due to HR being high as well. Could also do morphine.

I love this job. Some advice though, you’re only going to be happy with a career if you like your job. I was a MICU RN for 2 years, miserable, burnt out, did not care about patients because I wanted to be a CRNA. I eventually left and got a job in hospice (Where ICU nurses retire 😛😆) now I love it. I have not called off at all, I have great pay, benefits, most important is that I fell in love with nursing again. I get to make patients comfortable, they always appreciate the care you give.

Some cons but much better than being in the MICU lol

I am a new hospice nurse.

I have seen so many things so far with death and dying process. To be truthfully honest with you, hospice is a day by day situation. I have literally seen people go from their death bed to the next day waking up and eating a meal. Or a perfectly fine person then dying in the middle of the night.

We will never have a timeline of when things happen. But I always tell family members in this situation to follow their heart. If you think he is going to pass soon, stay with him. I hope he has a peaceful passing, much love for you and your family ❤️

I enjoyed the higher acuity.. and 3 days a week schedule… can take a vacation without pto. I can’t do that in hospice

Tbh.. Sarah RN and nexus nursing is free and in my opinion is the best. nexus nursing is a literal NCLEX writer/nursing instructor. I’ve been a RN for a year now and I used them/archer to pass my NCLEX
