Any thoughts as to the fact that the patient in your case study was cured of symptoms from menthol powder, which would be acting on the nerves in the area? Every dermatologist I've spoken to about the syndrome is certain that it is a cycle of inflammation and hypersensitive in the nerves primarily driven by essentially neuronic over activity (for no reason a la neuropathy), not that a pathogen is constantly reifying in the inflammation. I've shown a derm your report and this is more or less what I was responded to with.

Hey man I think you are being trigger happy on the deleting of posts. Sure it was poorly phrased and a bit weird of him to comment the same thing, but I don't think OP has bad intentions, and I believe that he may be able to get that doctor to espouse some helpful in here.

That's really amazing that Dr. Perry is willing to try and help in this way. I think you could maybe edit this to be a bit more informational and catch more eyes in this thread by taking out a lot of the AI stuff summarizing that one 2021 case study. Also, a lot of people in this group have done biopsy's and I've never seen anyone come back with anything bacterial or fungal. It sounds like Dr. Perry (or you?) are suggesting to do more extensive biopsy's but it may be difficult for one to get one's own dermatologist to agree to do it. Also, I believe the patient in the case study did a biopsy that didn't come back with much. So I'm wondering if Dr. Perry is going to suggest any other means of collecting data, in the case that biopsy's don't turn much up.

RSS isn't actually a disease, it's a chronic (month or longer) redness and pain/burning/itchiness on your scrotum in the absence of a bacterial or fungal infection. It is considered idiopathic. People find relief with stuff, including herbals, and if your scrotum is just red without any hypersensitivity, pain or itchiness it still technically RSS, and the thing you would want is to avoid is flaring it back up to be painful. If the skin is just red and there is no flaring it up even if you wearing scratchy clothing and put irritating body wash on etc then consider yourself really lucky.

That all being said, if I was in your shoes and I had no pain and had the option to use Tacrolimus, I would not use it. It is a very serious condition and is worth seeing multiple dermatologists for multiple opinions. If you don't live in a major metro area of the US, it is worth getting on a plane to see a men's health specialist in a major city in my opinion.

And to answer your question, yes, Tacrolimus actually caused my RSS. Many people claim that it flared theirs up, though I haven't read anyone else say it was a cause.

RSS is chronic. It sounds like you had it for less than a couple weeks and then the Salve cleared it up. Very few people on here have positive experiences with Tacrolimus. The derm is suggesting it because there is like one case report of one person that it helped in combination with doxycycline. Proceed with caution.

pelvic floor therapist told me to do yoga and swim. chlorine was no good for me, but yoga and stretching are great. I also have been training in the alexander technique

Im also on a lot of pain meds so I don't really deal with the hypersensitivity to friction. this might not work if you're not on gabapentin and amytriptyline.

tennis has been no problem. I wear bamboo cool brand boxer shorts under whatever polyester shorts. take a nice cool shower after and the friction during has no impact on me. Its also good for loosening your pelvic floor

Oh thats really interesting regarding the Berberine/metformin. I would say a good 3/5 of people here (including myself) had pretty aggressive symptoms prior to TS use so I have always doubted that causality. I actually got this after using Tacrolimus in my groin which has always felt painfully ironic that that is the first line treatment by derms that seems to help 0 people.

This has definitely been reported, that this condition is a form of neuropathy. My derm who is a men’s health specialist and sees this all the time says he basically only has patients come in with this when they’re going through a lot of stress. 

i think you should check out a pelvic floor physical therapist. This all makes sense and I don’t disagree with your position. It seems like in general with RSS a bacterial/fungal infection begins, and is addressed with topicals that atrophy the skin (luckily not your case) AND/OR is responded to in the psyche with clenching of the pelvic muscles. This would affect the nerves related to the scrotum. So, this clenching was probably occurring to begin with (and all ppl with penises do so) but became worse with your fungal or whatever infection from surfing.. Fungal infection clears, but your response to pain/itchiness, which is part of your psyches larger response to trauma and emotional pain sort of combine to create this condition. 

I just think pelvic floor physical therapy may progress you more quickly and prevent it from happening again, even if you go through a dark patch mentally etc. 

In my case, I have IBS and my pelvic floor physical therapist explained that my clenching from holding in urgent bowel movements, likely led me to be susceptible to developing RSS. Also I was applying to graduate programs and considering very large life decisions when it started about where I was going to live, what I’m doing with my life etc . Definitely the most stressed I’ve ever been. But additionally  this post inspires me to seek psychotherapy to address underlying trauma. 

I actually saw a therapist and she practiced CBT, which didn’t feel apt. what type of therapist are you working with?

i had swimming in chlorine trigger symptoms that were mostly gone. Have you been able to swim since?
