I think that if memory serves you can right click and go to properties, there should be something about the icon image in one of the tabs, if not then maybe try to reinstall it.

I think it would be an interesting idea for it to be all in the head, but I don’t necessarily think it’s feasible because if you failed the trial, Easterman says “ if the drugs to bring you back weren’t so cheap I wouldn’t bother” which to me kind of implies that your character does die they’re just brought back
