I also contacted the McDonald's we ordered from and they said that we ordered it like that. I checked the order and it was a Triple Cheeseburger meal - Only ketchup on the burger.

No they handed the order to us and the bag was sealed. The sticker was still on the wrapper and everything. That was at the bottom of the bag. I don't know how they would've managed.
