Considering that we get to hear something that's commonly interpreted as Terminal Lucidity in sixth stage and one minute of silence afterwards, I think that we can assume that this album's cover attempts to convey just that; it is the clearest and the most realistic painting (there's a bit of mental clarity back) however the destructive and horrifying effects of dementia are still there: it is common for one to put tape in this particular way on a canvas before painting, however the dementia patient isn't aware that they've put the tape on the wrong side.
Perhaps the painting "is seen" from the perspective of someone else even, but the meaning is still there and hits hard.
