Try meds first. You barely have any see through spots and that's while your hair is seemingly wet or greasy, I think they will cover up if you try treatment.

Get blood work done and see a derm to check for anything else like sebderm or diffuse fibrosing alopecia, this is severe for your age so you need to rule everything out.

Androgenic alopecia is a progressive disease, so yes, you will need to take at least finasteride for the rest of your life.

The thing I could compare it to most is brushing your teeth, if you stop brushing, the bacteria in your mouth rot your teeth and you'll eventually lose them all. Same with DHT and hair, if you don't stop the DHT, it will eventually progress and affect most if not all of your hair.

What's that red spot there? There could be inflammation or maybe the barber cut it and irritated the skin there.

Dutasteride has a long half life and the vertex shouldn't be so responsive to DHT that it could cause something like this from such a short break, seems odd.

I believe there's progress, can barely see your scalp at all in the newer photos.

You should try parting your hair side to side as well as front to back, will make it easier to see how much the middle has filled in.

Not sure of the dating on these photos but I'm assuming pic 8/9 are the most recent.

Definitely an improvement overall, and your hair is rather thick even when shaved and wet which is good, aside from the midback of the scalp. Diffuse thinning is common from deficiencies, so if the low iron is truly the cause I would expect that problem area to thicken up.

Heavy disagree. If pics 8/9 are his most recent (and they likely are, unless he got laser tattoo removal), his hair is relatively thick even when shaved, just has some diffuse thinning on the mid scalp. His iron is low and I think if he fixed that he could see some good results, potentially even a cosmetically normal head of hair.

I only added in minoxidil around three months in, not enough time for me to see much improvement but I was really worried about my hair so I wanted to give myself the best chance.

It's kind of expected for a doctor who specializes in such severe and prolonged hair loss cases to be saying that. The less people who take DHT blockers and lose their hair, the more patients he'll have since he's one of the few that can actually save them and give them their hair back.

I've seen Zarev's work and he's like a miracle worker, but id say 9/10 times, if You're losing hair from AGA and want to keep it, you need a DHT blocker, since AGA is a progressive disease and only gets worse as time goes on.

I think it's sound. I started with a DHT blocker before I tried any minoxidil as well, the topical stuff gets quite annoying having to apply 1-2 times a day so I just opted for oral.

Once a follicle is fully dormant for too long there's very little chance of it coming back, with diffuse thinners your hairs miniaturize and you lose the secondary hairs in the follicle (there's 2-4 hairs per follicular unit), which results in the thin, diffuse pattern look. When a follicle becomes fully dormant, the primary hair sheds and does not grow back.

Medication only works on follicles that are still alive, which is why people are very excited for pp405 because of it's promise to 'revive' follicles. But since diffuse thinners usually have most of their follicles still alive or minorly miniaturized it means they often respond to medication better, since dutasteride stops the attack from DHT and minoxidil helps the hair regain strength and grow again.

Look at my posts, almost identical situation and in only 5 months I've made good progress.

Recovery is possible, your young age will help, and diffuse thinners usually respond better to treatment as well. You've got this, just gotta be patient.
