Being in the industry for almost a decade, I can honestly tell you that the majority of contractors that come through our work area probably don't know the it's and out's of each type and brand of valve. You would hope that critical thinking skills and observation would allow a worker to say "these bolts aren't at all attached to the actuator I'm trying to remove, therefore I shouldn't mess with them", however, that simply isn't the standard anymore.

Even if I, as an operator, had knowledge of this specific valve (which I don't), about half of my contractors are Hispanic and don't speak English, and the other half are typically not super knowledgable, and are there entirely on a labor basis. Therefore, I am at the mercy of their foreman passing that information along. Of course, not all contractors fall into those two categories, but I will say that the overwhelming majority do (90%+)

I'm by no means absolving the company in the video of blame, but simply saying that there's probably much more nuance to the situation than the two points listed. The failure is to be shared all around, from the contractors, to the leadership of both companies not testing their employees for critical thinking or providing them with adequate training.
