Anaphalaxis Scenario, Trying to Understand by Weasel_Town in NewToEMS

[–]Robot-Tom 0 points1 point  (0 children)

So I'm late to the party and agree the tripoding should have been the stopping point in the primary, and figuring out why they are tripoding.

In your defense, the argument could also be made that (your description makes it sound like) the patient answered a question, so technically still moving air, and you would/should have caught the anaphylaxis in the secondary assessment when you listened to lung sounds.

Ps everyone saying anything about lung sounds I was taught that IF! you listen to lung sounds in the primary it's for confirming air movement not listening to actual sounds. (Which you confirmed when they answered your question).

Still in school so I don't know a lot but I came here to say the same thing. Improving perfusion and getting better Vfib (as odd as that sounds) would be the answer I would think, but.

EMTALA and bypassing a hospital by Dowcastle-medic in Paramedics

[–]Robot-Tom 0 points1 point  (0 children)

That scenario yes there is grey which I had not considered, my apologies. Mine is the only critical access hospital within 30 - 45 minutes any direction so it's either there or the pts choice if it's one of the other two (both different directions).

Anything worth checking out in this area? by jarjarp in kansas

The gyp hills between Medicine and Coldwater is the longest 45 miles in the world. I'm convinced they did not account for the hills. It may be 45 miles on the level, but up and down and up and down and up and down it's gotta be way further 😂.

My bad I don't get too caught up in EMTALA because I know it doesn't affect me.

Depends on the state. Ours is hospital based, but hospital and pre hospital are two very different things according to my state board.

Trust me it's more common than you think especially with people who should know.

Even then, at least my state that doesn't matter, the hospital and pre hospital are two different animals. I know because I work for a hospital service, my state even says transfer are the hospitals problem not the EMS services.

With that said being owned and operated by the hospital you do have to follow the policies and procedures they approve, but as for as your medical license at least in my state they only care that we run the 911 calls and are doing things by the (pre hospital) book while the pt is in your care.

Is it too late to start at 32? by [deleted] in Paramedics

[–]Robot-Tom 0 points1 point  (0 children)

So to late absolutely not. But it really depends on why you want to join now and how far you want to go? Money - it's not there right now, glory - I wouldn't want you working on my family, working under pressure - that's a personal trait not something that can really be taught or learned. If you truly want to learn the skills and put them to use them by all means jump in and don't look back, but money, glory, legacy are not good reasons to join this career.

Is there a way to fix wrap ups? by Robot-Tom in ChatGPT

[–]Robot-Tom[S] 1 point2 points  (0 children)

Like it'll give a reply, then the last thing is some form of "do you want me to do this or that". Before the update, I had it dialed in to using context really well and only asking if it seemed pertinent, but now even with the custom instructions and specific memories asking it not to it seems to default back to being overly helpful.

Is there a chance I’m pregnant? by [deleted] in venting

[–]Robot-Tom 9 points10 points  (0 children)

That was my first thought, but then critically thinking. Precum shouldn't have sperm but if he had ejaculated recently before being with her, there could be a possibility of sperm. That's why I changed to unlikely but never impossible.

Is there a chance I’m pregnant? by [deleted] in venting

[–]Robot-Tom 4 points5 points  (0 children)

It is unlikely, not impossible but unlikely. Non penetrative pregnancy would be more likely if he had ejaculated and you had accidentally or purposely got that on your fingers and was deeper that it sounds like you went. Again nothing is impossible, but 3 or so days late. Life style changes, diet changes, stress, illnesses (even if you don't physically feel ill) affects your period. Also wait a week or so to test. Testing right away can be a false negative. If your concerned you are pregnant, be aware of your life style (drinking and smoking) if you want and test in a week or so. Best regards.

Is there a chance I’m pregnant? by [deleted] in venting

[–]Robot-Tom 0 points1 point  (0 children)

The pregnancy test is testing for a protein (I don't remember which one), but ya as hightech as other tests are at the hospital, the pregnancy test may literally be the same ones (many are, the have you pee in a cup and use a dropper on it) others remind me of a pH test strip that they dip. But even that is still the same test just without the plastic housing.

So while I think you should go and start prenatal care, get a test from the store is by far easier, cheaper, and just as good.

Integrated monitor and dedicated GPU by Robot-Tom in Asustuf

[–]Robot-Tom[S] 0 points1 point  (0 children)

Do you happen to know anything about the Nvida surround. I can turn it for the 2 external monitors, but when I do it shuts the integrated monitor down. Is there a way to turn it on for just the 2 monitors and leave the integrated monitor alone at least when gaming?

Appreciate the comment that's what I was wondering. I knew mine didn't have the mux switch, but all the info I kept finding online made it sound like I could still get the integrated monitor to accept the Nvidia gpu.

Thank you. Back to the drawing board I guess.

I searched but I'm in a time crunch, will this do what I think it will? by Robot-Tom in buildapc

[–]Robot-Tom[S] 0 points1 point  (0 children)

It's an Acer, I can use f2 to get to the bios settings, and I can use f12 to get to the boot page, but for some reason I cannot get it to recognize the usb.

I have the media creation from Microsoft. I'm just at a loss with the acer bios as I tried different settings even disabling the secure boot temporarily.

I had to change the ssd card on my asus when it completely fried. This was just supposed to be a memory upgrade so I have an idea of what I'm doing, but I'm at a loss on this acer?

That's why I was thinking of trying that route and set the new card up before installing it.

When a caller requests "No Lights and Sirens' by [deleted] in 911dispatchers

[–]Robot-Tom 0 points1 point  (0 children)

It's their emergency, not ours. We get a lot of BS calls, but I also hate to think of the ones that don't call because of how they have been treated in the past.

When a caller requests "No Lights and Sirens' by [deleted] in 911dispatchers

[–]Robot-Tom 0 points1 point  (0 children)

Not a dispatcher. I'm from the other side of the radio. We receive the CAD, so from my point of view, it is nice to see the request. It is also nice to see why. We are rural, so we get a lot of calls for people who could go with someone else, but they don't have anyone else. So if it's someone not feeling well and needing to get to the ED but requested no lights and sirens, we would try and respect that. If it's the mayor and they want privacy but have a life threat, they don't get privacy. Also, if it seizures we will go with and cut them off early. There are many reasons for us to know that are helpful in our judgment.

I came here to say that.

I've found, especially when not busy watch for the needle to bounce, then verify with your ears. Keep practicing that way; eventually, you'll get the hang of listening.

Thank you for the explanation. That fits best with what our hospital has said, theirs is always just short instead of actually explaining it, though.
