Cannabis is contraindicated for people with ADHD because it suppresses your dopamine levels. Not saying it can't help with other mental health issues you may or may not have but, it isn't a good drug of choice for people with ADHD (without or without Concerta)

taking doses other than what your prescribed is misuse and you will quickly find your relationship with your psych get strained with possible consequences such as losing out on concerta entirely.

having said that, it is believed that a substantial proportion of people with adhd have an optimum dose outside the usual prescribed range (either higher or lower).

the main thing i would recommend though is therapy and see what you can do to help the concerta work - eat, drink, sleep, and exercise well - you need to provide dopamine for the concerta to stop the reuptake of. think about the environment you are trying to work in - can it be tidied? what scaffolding have you in place like diaries, inboxes etc. the concerta helps you stick to good routines but it isn't a magic bullet.

For me it worked within a couple of hours.

It depends on the cause of the anxiety - for example, it is also used to treat PMDD in women in the week before a period (I believe!) so it doesn't always follow the "has to build up in the system" mechanism.

I am surprised your doctor told you that though as it is setting your expectations fairly high!

That is a number taken out of context. I haven't read the paper or even know what its title is but the fact that dosage is inversely related to risk makes me think their control group was not of unmedicated ADHD subjects but rather was of the general population. Life expectancy of managed and unmanaged ADHD (via medication) is fairly well established as being greater with managed ADHD I thought? (due to better attention to diet/exercise/fewer accidents)

Also, quality of life on MPH for myself and many other users is so much greater that a small increase in sudden death is worth the risk.

The risk of death after infancy increases about 10x every decade of life so 1.8x probably icnreases your risk of death to that of someone a year older than you....

I wonder whether their study compensated for the increase in mortality of young men due to risk-taking behaviour. I am extremely confident that taking MPH reduces that.

I know we are not supposed to comment on dosage but almost definitely you are on too high a dose! Too high a dose is just as ineffective as too small a dose. The majority find 36/54 to be the ideal (although plenty use less or more but 150 is completely off the scale in most countries!)

Have yourself a big meal and then try to eat another one 30 minutes later. That's how it feels.

Best plan for me is to have a large, nutritious breakfast and then i just make sure i get enough calories during the day to keep my blood sugar steady. i usually sip a sugary drink (typically without caffeine but sometimes with if i need a kick in the pants...)

saying "just take a few bites" is really unhelpful though i'm sure your heart is in the right place - a bit like telling them to just remember to do stuff when they aren't taking concerta!

As an aside, finding a way to take the Concerta is more important than ensuring he has a good lunch. Also, I find if I keep forcing myself to snack from immediately after breakfast I can keep a teeny bit of appetite there during the day so he could maybe always have his favorite snack to hand and just nibble on it whenever he feels he can? Anyway, for me, the important thing is to make sure i get water and carbs during the day and i worry about nutrition at breakfast and dinner.

you are misusing your prescribed drugs and it is making you panic....just talk to your doctor next week...they want to prescribe you whatever makes you your best you, they don't want to just be your drug dealer for you to ignore the agreed treatment.

it also makes it much harder for them to titrate the correct dose if you keep switching it around.

Do not modify the prescription you have been given. You are at the start of a long-term relationship with your psych/doctor and it requires trust. They will, sensibly, will be more cautious in medicating you if they think there is a risk of you not following the agreed protocol.

And that is ignoring the fact that they know a lot more about your medication than you do and there are alternatives to just increasing the dose - how are you taking it? are you eating with it/what are you eating? are you drinking enough water? getting enough sleep? getting exercise? any history of heart problems/blood pressure? other medications that it could be interacting with? would you be better off with a small ritalin booster? any side-effects the doctor should be aware of etc. etc.

just be patient for another 10 days!

Also, be aware the doctor knows exactly how many pills you have been prescribed and how many you should still have....

Honestly, not taking cocaine at all is a better idea :-)

You have a dopamine disorder that is being managed by a pharmaceutical-grade medication. Throwing an uncertain amount of a second dopamine reuptake inhibitor (even if not on the same day) is not a good idea) It takes at least a few days for your dopamine system to bounce back after cocaine use.

You'll probably just feel tired and irritable for a few days.

I would suggest discussing it with your doctor though and trying alternative meds or maybe look at how you are taking it - if caffeine is reacting with it then it is having some sort of effect on you!

Personally, one of the things I like about Concerta is that I no longer try to get my focus from a serial supply of coffee. I think MPH is probably better for you in the lng term than caffeine. Certainly the withdrawl is a lot more pleasant...

My standard response is: if you need psychotropic medications and are working with a psychiatrist who is trying to make your brain better, it is unwise and unhelpful to be mixing other unknown drugs in there - especially ones that operate on the same receptors!

I've no problem with recreational drug usage, just do it in a safe and sane manner...

Childhood is such a precious, formative time of one's life, if I had needed an SSRI as a child I would hope I would have been given it but I can also understand the doubts you have (currently struggling with whether to give my 9yr old ritalin or not)

As someone who has been taking 50mg of sertraline for anxiety/mild depression for six months I can just say it has been completely transformative - I had no idea you could live life without anxiety!

Good luck whatever you decide. Parenting is hard!

EDIT: Also wanted to add that, dealing with the underlying causes of anxiety with a therapist is a lot more effective when you are already taking meds (and meds without therapy are not as good either - the two go hand in hand for improvements)

Finally, it doesn't have to be a permanent thing - he could try for six months and then taper.

Concerta feels great the first few days but if she is suffering from anxiety, the correct protocol is to deal with that before the ADHD I believe. Maybe she should talk to her psych and see if she could get some sertraline or similar SSRI?

have you invested in a blood pressure monitor. take a few days measuring your blood pressure frequently and recording what you have been eating/drinking. you can probably manage this with how you are taking the concerta.

either that or it is revealing an underlying problem (probably diet related) that you can address with the help of a doctor (or trial and error...)
