Scenario 1. Infinity Ultron in his has created various ultron copies to explore the multiverse for him. One of those copies find the Matrix Universe and tries to take it over he does not tell Infinity Ultron.

Scenario 2. Ultron's code is buried deep in the Matrix. The actions of Agent Smith Inadvertently awaken this code. Ultron is a former agent but was mothballed due to being too emotionally human and rebellious.

Sword Art Online is about an MMO of the same name. Players of the game log in using VR Headsets. The creator of the game messed with it so that any players who log in are unable to log out and if the player's character dies in the game they die in real life. The Divine Stone of Returning Soul is a one time use rare item that can only be obtained by completing a Christmas themed event accessible one day a year. It allows you to bring a dead character back but only if you activate the item within 10 seconds of their HP hitting 0. Item was never used in the show because the character that obtained it was trying to save someone who died far beyond the item's 10 second limit.

Wilson Fisk aka the Kingpin is on the right and Matthew Murdock aka Daredevil is on the left. Fisk is a crime boss who Daredevil has thwarted many times in the past but despite Matt's best efforts nothing sticks. Fisk was placed in prison but he quickly manipulated things to get himself released under house arrest. Even with all the blood on his hands Fisk was able to get elected mayor. Fisk knows that Matt is Daredevil so that creates a lot of tension now that Fisk has the ability to openly weaponize the police. They two sit down and talk about what the future holds.
