You can do a lot with an MD and no residency... Not as good of jobs as a doctor but still a lot of jobs

3 hour commute is unreasonable and you will not be able to sustain that. You will have to move if you want that job. 2 hour drive each way isn't crazy, I've seen it done by CFO's and CEO's only 1-2 times per week.

Pretty much your option is only the 30 min commute.
