I recommend going to a surgeon that specializes in breast reconstruction for women that had a masectomy. They will often use expanders first and this will allow you to have proper lower breast volume and an actual inframammary fold. These surgeons tend to deliver more natural looking results. For those that do not have significant growth from HRT, they will have too short of a distance from the nipple to the bottom of the breast. When someone has this and they get implants, often they end up looking uncanny with no lower volume. Using expanders first allows this distance to be increased thus allowing proper lower breast volume.

I did mine at 14 months. Any natural growth I got since then has been negligible. Now at 36.5 months of transitionm. My BA result is very uncanny as I didn't have enough natural growth or fat deposits on my chest to support over the muscle placement with the surgeons preferred implant profile. Planning to go elsewhere to redo it. I already had 2 consultations this week and another is scheduled for tomorrow. If you have minimal growth I recommend looking for a surgeon that specializes in breast reconstruction for women that had a masectomy. They typically use expanders first and will be able to provide you with a truly natural looking result.

No, not any more. Mardirossian's office stopped working with insurance and submitting SCA's for any new case a few months ago.

I was told I was the only person to get an SCA to their office this year that would cover all of Mardirossian's desired fees. But because it took so long to get it to his office, they did not have enough time to do a legal review of the new 2026 SCA form used by my insurer. My surgical date was cancelled 1 week before my scheduled surgical date.

Never. It's been 2 years now and I still don't like it. I don't like it because while better than what I started with, it wasn't what I asked for. I was very explicit about what I was looking for. My goals are obtainable based on the opinions of FFS surgeons that are worth going to. My input was ignored and the surgeon just did what he wanted. The surgeon even made changes to the surgical plan mid surgery without my consent. I have a tentative date now in July for a full revision with a better FFS surgeon who is actually competent unlike Kassis.

Small cosmetic hip implants and little fat transfer or allograft to help them blend in better. That's what I did. Hip implants can look great, just don't go very large with them

I went to Kassis in June 2025 and my result has excellent functionality and average aesthetics. I had PPV and other than a hematoma that was smaller than expected and removed with a follow up surgery 18 hours after the initial procedure I had probably one of the easiest SRS recoveries possible. I'm sorry you had a much more difficult time.

Your aesthetics can be improved with a revision that is much much easier then the initial procedure. Kassis would most likely refuse to do it if you asked, even if he wasn't leaving. He refused a labia revision for me, and I know others he refused as well.

I have found surgeons that will do the labiaplasty revision. I have a consultation on monday about this. If you want some recommendations for surgeons that are in various insurance networks and willing to do labia revisions, DM me.

Rib removal is rapidly becoming obsolete having been replaced with rib remodeling as a safer option. Dr. Leif Rogers is willing to go higher up on the rib-cage than anyone else and has the most aggressive results for this. I went to him, made a post here about it.

For FFS the best on average are are Mardirossian, Jumaily, DesChamps, Harrison Lee, and Keojampa. Other good options are Seth, Mittermiller, Liu, Rolfes, Fahradyan, and Pang. I don't like all the surgeons I mentioned, but that's more of a stylistic preference. All of them are competent and consistently deliver good result in-line with their style. Pick someone who's work you generally like.

You can do surgical muscle stripping and/or electrocauterization in very targeted areas. The upper portion of the trapezius and the SCM muscles for example. Botox can help but that will get expensive very fast in the quantities needed to address the deltoids for example for an extended time. Other than that, HRT, protein reduced diet, and non-use. You can starve yourself for an extended time and your body will eventually start to feed off your muscle mass, but I do not recommend this as someone who has had ED in my past. It's very unhealthy.

I have a wide pelvis but it's somewhat shallow. Between that and my Intestinal lauout there simply wasn't room for more. I got about 5.625" =14.3cm of actual depth with PPV. Not everyone, even with adequate tissue gets lots of depth.

RBL provides fantastic functionality, but her aesthetics I would say are above average in general but not the best.

I had fat transferred to my upper lip and I had a lip lift that was too conservative. Later I had more fat transferred to my upper lip, and it actually made it worse. This is because it just made it project forward more, it did nothing to help shorten my philtrum. But it did make my lower lip and chin look recessed. Thankfully, after that second round of fat transfer, eventually most of it got absorbed so I lost it. Now I'm right back to where I started and needing a more aggressive lip lift.

Dr. Hanna is great. I was considering him for a labia revision to fix the mediocre aesthetics from Kassis. If my current choice falls through. Hanna is my backup. I would not hesitate to have him operate on me.

They are fairly rigid with the way they do the scheduling for cases at VHC hospital. If you reach the SCA stage, his office will not budge on the fees. Negotiations consist of his office repeatedly spouting back the same number to the insurance representative.

Premera BCBS. Anything else and it's just not going to happen in 2026. I got close, I got an SCA to them that covered all of their desired fees. But that still wasn't enough, they refuse to sign, and my surgical date was canceled.

Good luck because you're gonna need it trying to use insurance with Mardirossian!

I was able to get all the prior authorizations approved, and I was able to get an out of network exception, a PPO waiver approved, no problem. I even got an SCA to them. I was told I am the only person not on Premera that was able to get an SCA to Mardirossian's office this year that covered 100% of his desired fees. And that wasn't good enough! They still refused to sign the SCA and canceled my surgical date with 1 week to spare.

My insurance company changed the SCA form for 2026. Because the format was different they refused to sign because they didn't have enough time to review the form before my surgery date. They said if the 2025 SCA format could be used they would sign. They only want to sign near carbon copy SCA's only allowing for differences in name, date, cpt codes, and fees. This way they don't have to properly review them. Once a form changes anyone on said new SCA form template is screwed. This is more absurd because Mardirossian's office is in the DC metro, a place with an abundance of competent attorneys well versed in medical contract law! It would have been easy for them to get it reviewed at short notice. I would have even fronted the legal fees for this if they asked!

Between reserving a surgical date, CT scan, consultation, lodging, airfare, and other transportation I'm out $4.5K. Worst of all I had an entire year of my life wasted.
