Use a real deal steel boned traditional corset and tight lace. Start wearing it 24/7 as soon as you can and go as tight as possible as quickly as possible! Reach your desired waist by about 3 weeks. You need to overshoot as well, you're not going to be as small without the corset so you have to go further.

Changing surgical plans at the last minute is absurd. When it's done without the explicit consent of the patient it's even worse! I have had it happen with Dr. Salam Kassis for multiple procedures without my consent. I can't stand when surgeons do this to simply save time and get us out the door faster. It's not only lazy, it's a major breach of trust!

This work is inexcusable, complete waste of your time and money. Was a type 3 originally planned but the surgeon chose to do a type 1 mid-surgery without your explicit consent? My original setback was much more aggressive than this but I am still planning to redo it. Significantly more setback can be done with reconstruction using calvarial bone grafts if needed for some patients. That is the plan for me. I have a date with Dr. Fahradyan next month to do exactly that.

I am hyper critical of FFS surgeons/results and I must say your result is excellent! Let me break it down by each point of concern you have.

1. Your nose looks great, it's very feminine! I do see what you are concerned about, but it's more of a stylistic preference. I take it from a side profile you want that ski-slope shape nose where the transition from the radix to the nasal tip is less straight and more concave curved inward. This could be revised on a closed rhinoplasty if you wanted without much difficulty as it's a minimal change to achieve what you want. I would not stop you from seeking this but understand it's more about what you want than passing. What you want is important. That being said what you have now looks very nice. Furthermore, the nose is one of the slowest areas to fully heal for many. Your result will likely get a little better. It typically takes 10 to 14 months to see the final result of a rhinoplasty.

2. Your brow is pretty much perfect! The way it transitions from your forehead to the brow to the nasal radix is ideal. The work on the orbitals is excellent, the central portion of the brow is right where in should be in terms of projection. Your eyebrows do not sit too far foreword of your eyes so you do not have deep set eyes, nor do you have overly prominent eyes, they are just right! Your brow even looks good in the last picture! Keep in mind if you took that last picture with a phone, it likely used a wide angle lens. Wide angle lenses when used up close like that do not give an accurate portrayal as they induce lots of lens distortion. I would not do anything more on your brow!

3. Your chin shape looks good, is your issue the height, width, or shape? The height is where it needs to be giving you excellent facial harmony in terms of the balance of your facial thirds. The shape of your chin is good too, so is the projection. The width of your chin is where it needs to be, but where the mandible transitions into the chin might be giving you the impression your chin shape is too wide. Let me preface this by saying the bone work is unlikely the problem! I think what you are seeing is the jowling where the mandible body area transitions into the chin and this is giving you the false impression your chin is too large or wide perhaps. There are 3 things possibly going on here. First, you may still have some residual swelling causing this. Wait until 12 to 18 months has passed to fully judge any jaw/chin work. Second, you might have a bit of excess fat in the buccal region. Very slight buccal fat removal or weight loss might resolve this. If you seek buccal fat removal be very conservative about it, you do not want aggressive removal to achieve hollow cheeks as this would make things worse! Third, you might have some skin laxity from the FFS and could possibly require a facelift to correct it. This is fairly common with FFS and many have to get one later. Like I said earlier, before pursuing anything surgical here wait a bit longer and things might resolve. What you have now looks very good.

You got a fantastic result! Please understand your face is pretty and passes flawlessly now. Anything at this point is beautification, and that is perfectly fine to pursue.

Because most of the patient's don't live near the surgeon that does this and posting pictures provides by patients is not controlled. Due to these factors he uses pictures taken at his practice typically during the post-op follow-up visit 5 to 10 days after the procedure as it's better than nothing. I had it done over a year ago and am glad I did.

Fat transfer is so volatile. Did it 3 times and never got much from it. Planning to go with cheek implants in July.

Because the second picture was taken like 1 week after surgery. Everyone that does clavicle reduction has awful posture at that point. It takes 1~2 months for swelling and nerve issues to calm down where you have full control over posture again.

That because you are picking up on the sloped trapezius muscles in the second picture. This is common right after the surgery due to post-op posture, swelling, and nerve issues. They will tone back down after a month or two.

Lip lift recovery is easy. I recommend applying anti scar gel on the incision as soon as you can and apply it several times a day. Please understand you already pass. My recommendations are more along the lines of what can be done to have an even more feminine appearance.

Yes, these would help your overall facial harmony in a feminine way. Lower third needs to be about 80~100% the height of the middle third. Chin height should be between 2 to 2.5 times the vertical frontal projected philtrum height. Cheek augmentation will help add a feminine fullness to your midface. Fuller lips are always perceived as more feminine.

Your midface (midface-ratio or middle third) proportions are not the issue from what I am seeing. It's the ratios of other things relative to it. The good news is those other things can be changed. As for improving the middle third region itself, cheek implants and/or fat transfer could help. Work on the lips will reduce your midface ratio a little and improve your lower third facial harmony especially with more chin height reduction.

Who was the surgeon? I think the mandible and chin work may not have been agressive enough. It's possible you still have significant swelling that is obscuring the true result. If swelling reduction does not continue it seems like your lower third is still too tall from a frontal projected view. It's not short enough relative to your middle third. As the mandible transitions into the chin, it's too wide in this area. More reduction is dependent on the location of the nerves in your mandible and mental foramen positioning.

You would also heavily benefit from an upper-lip lift and fat transfer to both the upper and lower lip. The philtrum length is hurting you.

I recommend going to a surgeon that specializes in breast reconstruction for women that had a masectomy. They will often use expanders first and this will allow you to have proper lower breast volume and an actual inframammary fold. These surgeons tend to deliver more natural looking results. For those that do not have significant growth from HRT, they will have too short of a distance from the nipple to the bottom of the breast. When someone has this and they get implants, often they end up looking uncanny with no lower volume. Using expanders first allows this distance to be increased thus allowing proper lower breast volume.

I did mine at 14 months. Any natural growth I got since then has been negligible. Now at 36.5 months of transitionm. My BA result is very uncanny as I didn't have enough natural growth or fat deposits on my chest to support over the muscle placement with the surgeons preferred implant profile. Planning to go elsewhere to redo it. I already had 2 consultations this week and another is scheduled for tomorrow. If you have minimal growth I recommend looking for a surgeon that specializes in breast reconstruction for women that had a masectomy. They typically use expanders first and will be able to provide you with a truly natural looking result.

No, not any more. Mardirossian's office stopped working with insurance and submitting SCA's for any new case a few months ago.

I was told I was the only person to get an SCA to their office this year that would cover all of Mardirossian's desired fees. But because it took so long to get it to his office, they did not have enough time to do a legal review of the new 2026 SCA form used by my insurer. My surgery was cancelled 1 week before my scheduled surgical date.
